定向软通道技术联合尿激酶冲洗对急性硬膜外血肿患者血肿吸收时间及并发症的影响

摘　　要：目的探讨定向软通道技术联合尿激酶冲洗对急性硬膜外血肿患者血肿吸收时间及并发症的影响,为急性硬膜外血肿的临床治疗提供参考。方法回顾性分析2012年2月—2017年4月该院神经外科诊治的21例急性硬膜外血肿患者的临床资料,患者均行定向软通道技术联合尿激酶冲洗治疗,纳为观察组,另将同期10例采用急性硬膜外血肿保守治疗的患者纳为对照组。治疗后,对比两组患者血肿吸收时间、住院时间、并发症。结果观察组患者血肿吸收时间、住院时间均明显短于对照组[血肿吸收时间(3.24±0.58)d vs (25.73±3.69)d;住院时间(10.16±2.23)d vs (20.35±3.87)d],并发症发生率明显低于对照组(0.00%vs 30.00%),差异有统计学意义(血肿吸收时间t=27.72,住院时间t=9.33,并发症发生率χ~2=6.98,P<0.05)。结论定向软通道技术联合尿激酶冲洗治疗可以显著缩短急性硬膜外血肿患者的血肿吸收时间,降低患者术后并发症,并有助于患者早日出院,是一项可以成功应用于临床并值得推广的新技术项目。Objective To study the effect of directional soft channel technology combined with urokinase irrigation on the absorption time of hematoma and complication of patients with acute epidural hematoma thus providing reference for the clinical treatment.Methods21cases of patients with acute extradural hematoma diagnosed and treated in our hospital from February2012to April2017were selected,and all patients were treated with urokinase irrigation on the absorption time of hematoma,regarded as the observation group,while10cases in the control group used the conservative treatment,after treatment,the absorption time of hematoma,length of stay and complications were compared between the two groups.Results The absorption time of hematoma,and length of stay in the observation group were obviously shorter than those in the control group[absorption time of hematoma[(3.24±0.58)d vs(25.73±3.69)d;length of stay(10.16±2.23)d vs(20.35±3.87)d],and the incidence rate of complications was obviously lower than that in the control group(0.00%vs30.00%),and the difference was statistically significant(absorption time of hematoma t=27.72,length of stay t=9.33,and the incidence rate of complicationsχ^2=6.98,P<0.05).Conclusion The directional soft channel technology combined with urokinase irrigation can obviously shorten the absorption time of hematoma,and reduce the postoperative complications,and it can contribute to the early discharge of patients,and it can be successfully promoted in clinic.
