摘  要:目的探讨小儿特发性矮身材采用重组人生长激素治疗的疗效和对骨代谢指标的影响。方法选取我院2016年6月~2018年5月收治的68例特发性矮身材患儿,按照随机数字表法分为对照组(34例)与观察组(34例)。对照组采用常规营养治疗(如补充适量的钙剂、维生素),观察组在对照组治疗方式基础上注射重组人生长激素(rhGH)。随访观察两组患儿第3、6、12个月疗效指标(身高(Ht)、体重(Wt)、生长速率(GV))及骨代谢指标(骨钙素(OC)、维生素D(VD))的变化。结果 (1)治疗3、6、12个月后两组体重比较,差异无统计学意义(P> 0.05),身高、生长速率比较观察组明显高于对照组(P <0.05);(2)治疗3、6、12个月后观察组患者OC指标与对照组比较生成增加明显(P <0.05),VD的生成两组比较差异并无统计学意义(P> 0.05)。结论采用重组人生长激素治疗小儿特发性矮身材,可以明显增加患儿的身高,骨钙素(OC)水平可以较好的反应生长速率,体内OC生成增加,骨合成指标升高,加快了骨骼的形成,促进骨生长。Objective To explore the curative effect of recombinant human growth hormone in treatment of in children with idiopathic short stature and its effect on bone metabolic state.Methods 68 children with idiopathic short stature who were admitted and treated in our hospital from June 2016 to May 2018 were selected.According to admission order,they were divided into the control group and the observation group according to random number table method,with 34 cases in each group.The control group was given conventional nutrition therapy(such as supplemented with appropriate amounts of calcium and vitamins)while the observation group was injected with recombinant human growth hormone(rhGH)on the basis of the control group.Changes of curative effect indexes(height(Ht),weight(Wt),growth rate(GV))and bone metabolic indexes(osteocalcin(OC),vitamins D(VD)of 3 months,6 months,and 12 months after treatment of children in two groups were followed up and observed.Results(1)There were no significant differences in weight of 3 months,6 months and 12 months after treatment between the two groups(P>0.05).Height and growth rate in the observation group were significantly higher than those in the control group(P<0.05).(2)After treatment for 3 months,6 months and 12 months,the OC index of the observation group was increased significantly compared with the control group(P<0.05).There were no significant difference in VD between the two groups(P>0.05).Conclusion Recombinant human growth hormone in treatment of in children with idiopathic short stature can significantly increase the height of children.Osteocalcin(OC)levels can better reflect the growth rate.The increase of OC in the body and bone synthesis index speed up the formation of bones and promote bone growth.
