改良简易负压吸引装置在食管癌术后难愈性切口中的应用研究

摘　　要：目的探讨改良创面负压吸引装置治疗食管癌术后胸部难愈性切口的临床效果。方法收集江苏省肿瘤医院胸外科2013年1月至2017年12月期间,食管癌手术后发生切口全层感染患者。将符合要求的研究对象60例患者分为试验组和对照组。采用传统高渗盐水治疗的30例为对照组(2013年1月至2015年12月期间);采用改良负压吸引装置治疗的30例为试验组(2016年1月至2017年12月期间)。比较两组患者的切口可行Ⅱ期缝合的时间、换药次数、换药费用。结果试验组患者的切口可行Ⅱ期缝合的时间为(8. 1±2. 5)d,换药次数为(7. 4±2. 2)次,换药费用为(1 500. 5±205. 8)元;对照组分别为(14. 4±4. 6)d、(15. 8±5. 1)次及(3 800. 4±480. 7)元,两组比较差异均有统计学意义(均P<0. 01)。结论使用自制负压吸引装置治疗胸部难愈性切口临床效果满意且更为经济实用,可以有效缩短创面愈合时间,降低换药费用。Objective To explore the clinical efficacy of modified wound vacuum suction device for the treatment of refractory chest incision after esophageal cancer surgery.Methods The patients who became a full-thickness infected after esophageal cancer surgery from the Thoracic Surgery Department of Jiangsu Cancer Hospital from January2013to December2017were selected and divided into experimental group and control group.Thirty patients treated with conventional hypertonic saline were included into control group(January2013to December2015).And30patients with wound infections treated with modified vacuum suction device were included into experimental group(January2016to December2017).The time required for stageⅡsuture,the number of dressing change,and the cost of dressing change were compared between the two groups.Results There were significant differences of the time required for stageⅡsuture([8.1±2.5]days vs[14.4±4.6]days),the number of dressing change(7.4±2.2vs15.8±5.1)and the cost of dressing change([1500.5±205.8]yuan vs[3800.4±480.7]yuan)between the patients in experimental group and control group(P<0.01).Conclusions The use of modified vacuum suction device for the treatment of chest refractory incision is practical with good efficacy,which can effectively shorten the wound healing time and reduce the cost of dressing change.
