摘  要:目的观察在骨肌肿瘤鉴别和诊断中应用MRI弥散加权成像的方法及价值。方法选取本院2014年6月至2017年7月期间收治的137例骨肌肿瘤患者为观察对象,所选患者均需在手术前进行肿瘤的良性与恶性鉴别与诊断;以ASSET技术对所有患者实施MRI弥散加权成像(DWI)检查,并分析观察不同b值之下的表观弥散系数(ADC)差异。结果病理证实良性肿瘤患者53例,恶性肿瘤患者84例,MRI弥散加权成像中良性肿瘤周围水肿6例,液化坏死10例,恶性肿瘤周围水肿31例,液化坏死55例,差异具有统计学意义(P<0.05)。DWI呈现出信号中,良性与恶性肿瘤患者也无统计学意义(P>0.05)。在弥散敏感系数b值定量下,良性肿瘤患者ADC值分别为1.96±0.51、1.77±0.42、1.68±0.45(×10-3 mm2/s),良性肿瘤患者ADC值分别为1.24±0.44、1.12±0.37、0.98±0.37(×10-3 mm2/s),差异存在统计学意义(P<0.05)。结论骨肌肿瘤对鉴别和诊断中应用MRI弥散加权成像的有重要作用。Objective To observe the method and value of MRI diffusion weighted imaging in the identification and diagnosis of musculoskeletal tumors.Methods A total of137patients with osteomyoma admitted and treated in our hospital from June2014to July2017were selected as the observation objects.All the selected patients were required to conduct preoperative benign and malignant tumor differentiation and diagnosis.MRI dispersion weighted imaging(DWI)examination was performed on all patients with ASSET technology,and differences in apparent dispersion coefficient(ADC)under different b values were analyzed and observed.Results There were53cases of benign tumor and84cases of malignant tumor,6cases of benign tumor edema,10cases of liquefaction necrosis,31cases of malignant tumor edema,and55cases of liquefaction necrosis in MRI diffusion weighted imaging.The difference was statistically significant(P<0.05).There was no statistically significant difference in DWI signal between benign and malignant tumor patients(P>0.05).Under the quantitative value of diffusion sensitivity coefficient b,the ADC values of patients with benign tumors were1.96±0.51,1.77±0.42,and1.68±0.45(×10^-3mm^2/s),respectively,and the ADC values of patients with benign tumors were1.24±0.44,1.12±0.37,0.98±0.37(×10^-3mm^2/s),with statistically significant differences(P<0.05).Conclusion Musculoskeletal tumors play an important role in the differentiation and diagnosis of musculoskeletal tumors by MRI diffusion weighted imaging.
