知柏地黄丸联合盐酸米诺环素软膏治疗慢性牙周炎55例疗效评价

摘　　要：目的探讨知柏地黄丸联合盐酸米诺环素软膏治疗慢性牙周炎(CP)的临床疗效及安全性。方法选取医院2015年1月至2017年12月收治的CP患者109例,按随机数字表法分为对照组(54例)和治疗组(55例)。两组患者均给予盐酸米诺环素软膏治疗,治疗组患者加用知柏地黄丸,均连续治疗4周。结果治疗组患者总有效率为94. 55%,显著高于对照组的77. 78%(χ~2=6. 455,P=0. 011);两组患者的血清白细胞介素6(IL-6)、白细胞介素1β(IL-1β)、C反应蛋白(CRP)及肿瘤坏死因子-α(TNF-α)水平均明显低于治疗前,且治疗组均明显低于对照组(P <0. 05);两组牙周袋深度(PD)、牙周附着水平(AL)、龈沟出血指数(SBI)及菌斑指数(PLI)均明显低于治疗前,且治疗组均明显低于对照组(P <0. 05);不良反应发生率治疗组为5. 45%(3/55),对照组为7. 41%(4/54),两组比较无明显差异(χ~2=0. 173,P=0. 678)。结论知柏地黄丸联合盐酸米诺环素软膏治疗CP临床疗效显著,能明显减轻机体炎性反应,改善牙周炎相关指标,且安全性较好,值得临床推广。Objective To investigate the clinical effect and safety of Zhibai Dihuang Pills combined with Minocycline Hydrochloride Ointment in the treatment of chronic periodontitis(CP).Methods Totally109CP patients admitted to our hospital from January2015to December2017were selected and divided into the control group(54cases)and the treatment group(55cases)according to the random number table method.The patients in the control group were treated with Minocycline Hydrochloride Ointment,while the patients in the treatment group were treated with Zhibai Dihuang Pills combined with Minocycline Hydrochloride Ointment.Both groups were treated for4weeks.Results The total effective rate of the treatment group was94.55%,which was significantly higher than77.78%of the control group(χ^2=6.455,P=0.011).The levels of serum interleukin-6(IL-6),IL-1β,C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α)in the two groups were significantly lower than those before treatment,and the levels of serum IL-6,IL-1β,CRP and TNF-αin the treatment group were significantly lower than those in the control group(P<0.05).The periodontal pocket depth(PD),periodontal attachment level(AL),sulcus bleeding index(SBI)and plaque index(PLI)of the two groups were significantly lower than those before treatment,and those in the treatment group were significantly lower than those in the control group(P<0.05).The incidence rate of adverse reactions was5.45%(3/55)in the treatment group and7.41%(4/54)in the control group,there was no significant difference between the two groups(χ^2=0.173,P=0.678).Conclusion Zhibai Dihuang Pills combined with Minocycline Hydrochloride Ointment in the treatment of CP have significant clinical effect,can significantly reduce the body's inflammatory reaction,improve the indicators related to periodontitis,and have good safety,which are worthy of clinical promotion.
