生长抑素与奥美拉唑治疗重度急性有机磷中毒合并急性胰腺损伤25例分析

摘　　要：目的:探讨生长抑素联合奥美拉唑治疗重度急性有机磷农药中毒(AOPP)合并急性胰腺损伤的效果。方法:收治重度急性有机磷合并急性胰腺损伤患者50例,随机平分两组,每组各25例。所有口服有机磷农药6 h内患者常规给予洗胃、导泄、血液灌流、阿托品、复能剂、禁食水、胃肠减压、维持水电解质及酸碱平衡等治疗。对照组给予奥美拉唑治疗,试验组在给予奥美拉唑基础上加用生长抑素,观察两组治疗效果。结果:试验组总有效率96.0%,优于对照组的84.0%。试验组在肠道功能恢复、症状缓解、血清淀粉酶下降、乙酰胆碱酯酶(ACHE)恢复时间方面均明显优于对照组,差异有统计学意义(P<0.05)。结论:生长抑素与奥美拉唑治疗中毒急性有机磷中毒合并急性胰腺损伤疗效显著。Objective:To investigate the effect of somatostatin combined with omeprazole in the treatment of severe acute organophosphorus pesticide poisoning(AOPP)with acute pancreatic injury.Methods:50patients with severe acute organophosphorus combined with acute pancreatic injury were selected,they were randomly divided into the two groups equally.All patients were given gastric lavage,catheterization,hemoperfusion,atropine,energizer,fasting water,gastrointestinal decompression,water electrolyte maintenance and acid-base balance within6hours.The control group was treated with omeprazole,and the experimental group was treated with somatostatin on the basis of omeprazole.Results:The total effective rate of the observation group was96%,which was better than84%of the control group.The recovery time of intestinal function,remission of symptoms,decrease of serum amylase and acetylcholinesterase(ACHE)in the experimental group were significantly lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Somatostatin and omeprazole are effective in the treatment of acute organophosphorus poisoning combined with acute pancreatic injury.
