H-UPPP治疗重度OSAHS手术疗效分析

摘　　要：目的探讨H型腭咽成形术(H-palatopharyngoplasty, H-UPPP)治疗重度阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome, OSAHS)的疗效。方法 20例术前检查无手术禁忌证的腭咽平面阻塞重度OSAHS患者,行H-UPPP手术治疗,术后随访至少6个月。比较分析手术前后各相关参数的变化。结果 AHI由术前的(50.47±20.954)次/小时下降至术后的(32.53±23.36)次/小时,最低血氧饱和度由术前的(59.35±13.72)%上升至术后的(70.42±12.44)%,ESS由术前的(18.15±3.60)下降至术后的(11.15±2.83),鼾声量表评分由术前的(8.00±1.25)下降至术后的(3.45±1.23),P均<0.01。结论H-UPPP是治疗腭咽平面阻塞重度OSAHS的有效手术方式。Objective To observe the therapeutic effect of H-palatopharyngoplasty(H-UPPP)on cases with severe degree of OSAHS based on a clinical trial.Methods Included ion this study were 20 patients with severe degree of OSAHS(AHI>30),with the obstructive lesions at the level of oropharyngx,treated by the procedure of H-UPPP in our Hospital.All these patients were followed up at least for6months postoperatively.Then,analyzed were the parameters associated with the therapeutic effects collected before and after the operation respectively in a comparative way.Results As evaluated by the end of following up period,AHI was decreased from(50.47±20.954)to(32.53±23.362)following the operation,while the lowest oxyhemoglobin saturation(LSaO2)was increased from(59.35±13.72)%to(70.42±12.44)% and ESS was reduced from(18.15±3.60)to(11.15±2.83)after the surgery,with all the P values being<0.01.Conclusion H-UPPP is effective for the treatment of severe degree of OSAHS with the lesions at the level of oropharyngx.
