摘　　要：目的观察慢性乙肝合并非酒精性脂肪肝患者CD8^+T细胞与胰岛素抵抗的关系。分析与慢性乙型肝炎患者合并脂肪肝T细胞亚群差异,及其与胰岛素抵抗可能的相关性。方法不同程度的慢性乙肝合并非酒精性脂肪肝患者(NFLD)60例和慢性乙型肝炎(CHB)50例,健康体检者(Control)20例,流式细胞分析T淋巴细胞亚群(CD3、CD4、CD8)的比例,并分析各组体重指数(BMI)、血清转氨酶(ALT)、低密度脂蛋白(LDL-C)、总胆固醇(TC)、胰岛素抵抗(HOMA-IR)情况。结果慢性乙肝患者、慢性乙肝合并非酒精性脂肪肝患者CD3^+、CD4^+、CD8^+表达水平与健康组比较下降。CD8^+、CD4^+/CD8^+与健康对照组比较差异具有统计学意义。慢性乙肝合并非酒精性脂肪肝胰岛素抵抗明显高于慢性乙型肝炎组、健康组。结论慢乙肝合并脂肪肝组胰岛素抵抗高于慢性乙肝组,与CD8^+T细胞下降具有相关性。Objective To analyze the relationship between T cells and antiviral response in chronic hepatitis B patients with non-alcoholic fatty liver disease.Methods 60 patients with different levels of non-alcoholic fatty liver disease and chronic hepatitis B,50 patients with chronic hepatitis B,and 20 Healthy volunteers(Control)were enrolled for the study.Flow cytometry analysis was used for detecting T lymphocyte subsets(CD3,CD4,CD8)ratio.The analysis of several indexes,including BMI,ALT,LDL-c,TC and HOMA-IR was also conducted.Results Compared chronic hepatitis B patients,chronic hepatitis B patients with non-alcoholic fatty liver patients with the control group,the CD3^+,CD4^+/CD8^+expression levels were significantly lower.In chronic hepatitis B combined with non-alcoholic fatty liver patients,the insulin resistance was significantly higher than those of chronic hepatitis B and healthy groups.Conclusion The insulin resistance of the chronic hepatitis B plus fatty liver group was higher than that of the chronic hepatitis B group,and it has a correlation with CD8^+T cells decreasing.
