壹期煤工尘肺患者血清KL-6水平测定及意义

摘　　要：目的探讨壹期煤工尘肺(CWP)患者血清涎液化糖链抗原(KL-6)水平变化及其意义。方法采用化学发光法检测72例壹期CWP患者(CWP组),37例与CWP组相同接尘条件的井下接尘矿工(接尘组)及43例非接尘井上健康查体人员(正常组)血清KL-6水平。结果血清KL-6水平在正常组、接尘组及CWP组3组间依次呈升高趋势,但差异无统计学意义(P>0. 05);在不同接尘年限、不同接尘性质、不同肺功能及是否并发症等临床相关指标间比较,差异也均无统计学意义(P>0.05);KL-6与1s用力呼气量(FEV1%)、用力肺活量(FVC%)、用力呼气1s率(FEV1/FVC%)均无相关关系(r=0.119、0. 107、-0.006,P>0.05)。结论血清KL-6水平检测可能不适于壹期CWP肺纤维化早期诊断和预测。Objective To explore the changes and the significance of serum Krebs von den Lungen-6(KL-6)levels in the patients with stagecoal worker’s pneumoconiosis(CWP).Methods The levels of serum KL-6 were determined in 72 stage CWP patient(CWP group),37 healthy miner(dust-exposed group)and 43 healthy controls(normal group)by chemiluminescence immunoassay(CLIA).Results Serum KL-6 levels showed rising tendency innormal group,dust-exposed group and CWP group,and no significant differences were found among these three groups(all P>0.05).There was no obvious difference of KL-6 levels in two time of exposure groups,two kinds of exposure groups and simple CWP and CWP with complication patient groups respectively,and there were no significant differences among three lung function subgroup(all P>0.05).There was no correlation between the levels of KL-6 and FEV1(%),FVC(%)and FEV1/FVC(%).Conclusion Serum KL-6 levels might be inappropriate to the prevention and diagnosis in stage CWP earlier.
