盆底肌按摩对行冻融胚胎移植周期薄型子宫内膜患者妊娠结局的影响

摘　　要：目的:观察盆底肌按摩联合人工周期对行冻融胚胎移植周期(FET)薄型子宫内膜厚度、类型、血流以及妊娠率的影响。方法:将准备行FET的薄型子宫内膜患者60名,随机分为两组,每组30例。两组患者均给予人工周期(月经第3天起服用戊酸雌二醇片(补佳乐,拜耳医药有限公司)早晚各3毫克,连用12天后添加黄体支持)。观察组经净后,每天配合盆底肌按摩,时间为15min,频率为50Hz,选穴为会阴、长强、八髎穴,直至添加黄体酮日;对照组则单纯人工周期。比较两组患者黄体酮日内膜厚度、类型、血流和临床妊娠率。结果:治疗后:(1)经过盆底肌按摩治疗后,观察组比对照组黄体酮日内膜Ⅰ型血流明显减少,Ⅱ+Ⅲ型血流明显增加,差异具有统计学意义(P <0. 05);(2)盆底肌按摩器治疗后,添加黄体酮日观察组内膜厚度明显高于对照组,差异具有统计学意义(P <0. 05);(3)经盆底肌按摩后,添加黄体酮日观察组A型内膜例数高于对照组,但差异无统计学意义(P> 0. 05);(4)临床妊娠率观察组明显高于对照组,差异具有统计学意义(P <0. 05)。结论:雌孕激素联合下人工周期配合盆底肌按摩能够改善行FET周期的薄型子宫内膜患者的内膜厚度及血流,提高患者的妊娠率。Objective:To observe the effect of pelvic floor muscle massage combine with artificial cycle on endometrium and pregnancy rate in frozenthawed embryo transfer(FET)cycles of patients with thin endometrium.Methods:60 patients with thin endometrium receiving FET were selected and divided into observation group and control group randomly,with 30 cases in each group.The observation group received pelvic floor muscle massage combine with artificial cycle.The control group received artificial cycle only.The patients all took estradiol valerate at the beginning of the third day of the menstrual cycle,3mg/time,twice a day.After 12 days,luteal support was added.The observation group received pelvic floor muscle massage for 15 minutes per day after a clean period of menstruation until adding luteal support.The massage frequency was 50Hz,and the acupoints were Huiyin,Changqiang and Baliao.The endometrium thickness,type and blood flow on the day of adding luteal support and clinical pregnancy rate of the two groups were observed.Results:1)After the pelvic floor muscle massage,the type I endocardial blood flow was decreased and type II and III were improved significantly in observation group than that in control group,with statistically significant difference(P<0.05).2)After pelvic floor muscle massage,the endometrium thickness in observation group on the day of adding luteal support was increased significantly than that in control group,with statistically significant difference(P<0.05).3)After the pelvic floor muscle massage,the number of type A endometrium in observation group was higher than that in the control group,but there was no statistically significant difference(P>0.05).4)The clinical pregnancy rate in observation group was obviously higher than that of control group with statistically significant difference(P<0.05).Conclusions:The pelvic floor muscle massage combine with artificial cycle can improve the endometrial thickness,endometrial blood flow and pregnancy rate of patients with thin endometrium.
