摘 要:目的:了解北京地区近年来支原体的耐药情况,为临床合理使用抗生素提供依据。方法:对1020例支原体感染病例进行回顾性分析,统计其对12种常见抗生素的耐药情况。结果:对美满霉素、交沙霉素、四环素、强力霉素的耐药率很低,均小于3%;对环丙沙星、左氧氟沙星、氧氟沙星、司巴沙星的耐药率较高,分别达到84. 51%、51. 18%、44. 51%、39. 12%;对红霉素、阿奇霉素、罗红霉素、克拉霉素,产生了一定的耐药,分别为21. 67%、19. 31%、18. 63%、18. 73%。但根据支原体类型分组分析却发现,Uu感染组对红霉素、阿奇霉素、罗红霉素、克拉霉素的耐药率均小于9%,而Uu合并Mh感染组、Mh感染组对这4种抗生素的耐药率基本大于80%;对其他8种抗生素的耐药率,各组差异不大。根据标本来源部位分析,各部位的支原体耐药率无明显差异。结论:北京地区支原体耐药率相对较低,感染部位的不同可能不会影响耐药情况,美满霉素、交沙霉素、四环素、强力霉素可以考虑作为首选药物,而单纯Uu感染患者也可以选用红霉素、阿奇霉素、罗红霉素、克拉霉素。Objective:To investigate the drug resistance condition of mycoplasma in Beijing, in order to provide evidence for the use of antibiotics in clinic treatment.Methods:Retrospective analysis was performed on 1020 cases of mycoplasma infection,to calculate the drug resistance condition of 12 common antibiotics.Results:The drug resistance rate of minocycline,tetracycline,doxycycline and josamycine was very low,less than 3%.The drug resistance rate of CPFX,levofloxacin,ofloxacin and sparfloxacin was high,up to 84.51%,51.18%,44.51% and 39.12% respectively.The drug resistance to erythrocin,azithromycin,roxithromycin and clarithromycin,was 21.67%,19.31%,18.63% and 18.73% respectively.However,according to mycoplasma type grouping analysis,drug resistance rate of erythrocin,azithromycin, roxithromycin and clarithromycin in Uu infection group was less than 9%.The drug resistance rate of these four antibiotics in Uu combined Mh infection group and Mh infection group was more than 80%.There was no significant difference between groups in the drug resistance of other 8 antibiotics.There was no significant difference in the drug resistance of different parts of body according to the analysis of sample source.Conclusions:Mycoplasma resistant rate in Beijing is relatively low.The infection position may not have relationship with resistance rate.Minocycline,josamycine,tetracycline and doxycycline can be considered as preferred drugs and for Uu infection,erythromycin,azithromycin,roxithromycin and clarithromycin can also be used.
