摘  要:目的分析大鼠狭长窄蒂皮瓣术前局部皮下连续注射重组人生长激素(rh GH)对皮瓣成活、血管内皮生长因子(VEGF)和CD34表达及微血管的影响。方法随机选取30只健康SD大鼠并分成研究组和对照组,各15只,其中研究组大鼠于术前3 d行连续背部右侧拟皮瓣区局部皮下注射rh GH,对照组仅于手术当天予皮瓣区局部皮下注射rh GH,两组大鼠均于右侧皮瓣区制作一狭长窄蒂皮瓣模型。术后1周,比较两组大鼠的皮瓣成活面积;并通过酶联免疫吸附试验和免疫组织化学法测定两组大鼠的VEGF和CD34表达,同时比较两组大鼠皮瓣微血管密度。结果术后1周,研究组大鼠的皮瓣成活面积百分率显著高于对照组(P <0. 05);同一时间点下,研究组大鼠的VEGF、CD34表达水平、皮瓣微血管密度均较对照组更高(P <0. 05)。结论大鼠狭长窄蒂皮瓣术前局部皮下连续注射rh GH有利于提高皮瓣成活率,改善VEGF及CD34表达水平,促进微血管生成,降低皮瓣缺氧、缺血的影响。Objective To analyze the effects of local continuous subcutaneous injection of recombinant human growth hormone(rhGH)on the survival of the flap,the expression of vascular endothelial growth factor(VEGF)and CD34 and the microvasculature in the skin flap of the rat.Methods 30 healthy SD rats were randomly selected and divided into 15 rats in the study group and the control group,of which the rats in the study group were subcutaneous injection of rhGH in the right flaps area of the right side of the back of the back in 3D before operation,and the control group was given the subcutaneous injection of rhGH in the skin flap area only on the day of the operation.The two groups of rats were made of a narrow and narrow pedicle flap model in the right side flap area.1 week after operation,the survival area of the flaps in the two groups of rats was compared,and the expression of VEGF and CD34 in the two groups of rats was measured by enzyme linked immunosorbent assay and immunohistochemistry,and the microvascular density of the skin flaps in the two groups was compared.Results 1 week after operation,the percentage of survival area of the skin flap in the study group was significantly higher than that of the control group(P<0.05).At the same time point,the expression level of VEGF and CD34 and the microvascular density of the flap were higher in the study group than in the control group(P<0.05).Conclusion The continuous subcutaneous injection of rhGH before the operation of narrow and narrow skin flap in rats is beneficial to improve the survival rate of the skin flap,improve the expression of VEGF and CD34,promote the formation of microvasculature,reduce the effect of anoxia and ischemia of the skin flap.
