老年住院患者衰弱状态评估

摘　　要：目的评估老年住院患者衰弱状态,探讨衰弱与营养状况、功能状态以及共病等的相关性。方法回顾性选取2018年1月至2018年6月于首都医科大学附属北京友谊医院老年内科住院的老年患者(年龄≥60岁) 227例,应用Fried衰弱表型及FRAIL量表进行衰弱评估。结果 227例老年患者中,男性156例,女性71例;平均年龄80. 51±9. 66岁(60岁-99岁))。衰弱患病率分别为54. 19%(123/227)(Fried衰弱表型)、39. 65%(90/227)(FRAIL量表)。衰弱患者相较于无衰弱患者,其年龄、共病指数以及营养不良发生风险更高,日常生活活动量表(ADL)评分、工具性日常生活活动量表(IADL)评分、体重指数(BMI)均明显下降,差异有显著性(P <0. 001)。Fried衰弱表型和FRAIL量表评估衰弱结果一致性较高,Kappa值为0. 590(P <0. 001)。结论衰弱在老年住院患者中发生率高,与年龄、营养状态、共病、功能状态等具有相关性。Objective To evaluate frailty status in elderly inpatients and analyze correlation of malnutrition,physical function and comorbidity with frailty. Methods 227 inpatients older than 60 years in geriatric ward of Beijing Friendship Hospital from January 2018 to June 2018 were retrospectively enrolled. Frailty was defined according to Fried frailty phenotype and FRAIL scale. Results Patients age was( 80. 51 ± 9. 66)years,156 males and 71 females. The prevalence of frailty was 54. 19%( 123/227,Fried frailty phenotype) and 39. 65%( 90/227,FRAIL scale). Patients with frailty were of higher age,higher comorbidity index,higher proportion of malnutrition,impairment of activities of daily living( ADL) and instrumental activities of daily living( IADL),lower body mass index( BMI)( P < 0. 001). The results of Fried frailty phenotype and FRAIL scale were consistent with each other( kappa = 0. 590,P < 0. 001). Conclusion Frailty has high prevalence in elderly inpatients and it is related with age,malnutrition,comorbidity and function status.
