摘  要:目的分析双上肢收缩压差(IASBPD)与肱踝脉搏波传导速度(ba-PWV)的关系。方法选取2007年9~12月在北京石景山区苹果园社区卫生服务中心进行临床调查的1 424名研究对象,采用无创疾病筛检仪VP1000进行双上肢血压同步测量。IASBPD定义为左上肢收缩压与右上肢收缩压差值的绝对值,IASBPD≥10 mm Hg定义为IASBPD增高,并测量ba-PWV、记录其他心血管危险因素。利用单因素和多因素logistic回归分析IASBPD增高与ba-PWV增快(ba-PWV≥1 800 cm/s)的关系。结果 IASBPD增高组的ba-PWV较IASBPD正常组明显增快[(1 723. 0±483. 0) cm/s比(1 527. 0±386. 0) cm/s,P <0. 05]。多因素logistic回归分析结果显示IASBPD增高与ba-PWV增快呈正相关(OR=1. 65,95%CI:1. 03~2. 62,P=0. 04)。结论 IASBPD增高与ba-PWV增快独立相关。Objective To analyze the relationship between inter arm systolic blood pressure difference (IASBPD) and brachial-ankle pulse wave velocity (ba-PWV). Methods A total of 1 424 individuals from Ping Guoyuan Community Health Service Center of Shi Jingshan District in Beijing were involved during September to December, 2007. The blood pressure was assessed in both arms simultaneously by VP-1 000, and IASBPD was defined as the absolute difference value of left and right upper arm systolic blood pressure. IASBPD ≥10 mmHg was defined as increase of IASBPD. ba-PWV was measured by VP-1 000 , and other cardiovascular disease risk factors were also recorded. The relationship between the increase of IASBPD and the increase of Ba-PWV (≥1 800 cm/s ) was analyzed by Logistics regression method. Results ba-PWV in IASBPD≥10 mmHg group was significantly faster than IASBPD<10 mmHg group [(1 723.0 ± 483.0) cm/s vs.(1 527.0 ± 386.0) cm/s, P<0.05]. Multivariate logistics regression showed the IASBPD≥10 mmHg was positively correlated to ba-PWV ≥1800 cm/s ( OR =1.65, 95% CI: 1.03-2.06, P<0.05) independently. Conclusions IASBPD ≥10 mmHg was positively correlated with ba-PWV ≥1 800 cm/s independently.
