摘  要:目的采用二维经胸超声心动图的方法评估健康人群的升主动脉内径大小,分析其影响因素。方法入选2013年5~11月在北京医院体检中心体检的2 039名健康志愿者。取胸骨旁左心室长轴切面,在主动脉窦上缘1. 0 cm处垂直于血管壁,从回声内缘至内缘测量主动脉内径,或测量升主动脉近端的最大径线。结果入选的2 039名健康受试者,男性1 267例(62. 1%),女性772例(37. 9%),年龄18~89岁,平均(46. 9±12. 3)岁。升主动脉的平均内径为(29. 4±3. 7) mm。对升主动脉内径有显著影响的因素有性别(P <0. 001)和年龄(P <0. 001),但身高、体重、体质指数、收缩压和舒张压等无明显影响(均为P> 0. 05)。校正体质指数、收缩压和舒张压后,年龄和性别是升主动脉内径的独立影响因子(均为P <0. 001)。同时,男性的升主动脉内径显著高于女性[(30. 4±3. 5) mm比(27. 7±3. 5) mm,P <0. 001]。随年龄增加,升主动脉内径逐渐增加(P <0. 05)。此外,女性受试者除18~29岁组和30~39岁组之间及60~69岁组和70岁以上组之间无显著差异外(均为P> 0. 05),其他各组间均有显著差异(均为P <0. 001)。男性受试者除了60~69岁组和70岁以上组之间无显著差异外(均为P> 0. 05),其他各组间均有显著差异(均为P <0. 001)。结论经胸超声心动图是测量升主动脉内径的常用手段,性别和年龄对升主动脉内径有显著影响,应根据不同性别和年龄制定参考值。Objectives To assess dimensions of the ascending aorta in a large group of healthy volunteers and analyze its influential factors using two-dimensional transthoracic echocardiography. Methods 2 039 healthy volunteers were recruited from May 2013 to November 2013 in Beijing Hospital Physical Examination Center. Aortic dimensions were obtained following predefined protocols in parasternal long-axis view using the inner-edge-to-inner-edge measurement. Diameters were measured at 1.0 cm above the sinuses of Valsalva, or the maximum diameter of the proximal tubular ascending aorta. Results A total of 2 039 healthy volunteers were enrolled in this study, including 1 267 men (62.1%) and 772 women (37.9%). Mean age was (46.9±12.3) years with a range of 18-89 years. The mean dimension of the ascending aorta was (29.4±3.7) mm. Gender ( P<0.001) and age ( P<0.001) had significant effect on dimensions of the ascending aorta, while height, body weight, body weight index (BMI), systolic blood pressure and diastolic pressure had no significant effect (all P>0.05). Gender and age were independent influence factors after adjusting BMI, systolic blood pressure and diastolic pressure. Men showed significantly larger aortic dimensions when compared with women [(30.4±3.5) mm vs.(27.7±3.5) mm, P<0.001]. Dimensions strati cation by age ranges was displayed. In multiple comparison analysis, aortic dimensions significantly increased in different age group ( P<0.05). Subjects of different genders were compared by age groups respectively.Results showed that aortic dimensions increased with age in both genders group. In female group, compared with 18-29 year-old group, aortic diameters were larger than 30-39 year-old group without statistical significance. And the same was found between 60-69 year-old group and over 70 year-old group ( P>0.05).
