中药联合体外冲击波治疗老年早期酒精性股骨头坏死研究

摘　　要：目的:观察内服外敷中药联合体外冲击波治疗老年早期酒精性股骨头坏死的疗效及安全性。方法:将60例96髋老年早期酒精性股骨头坏死患者随机分为治疗组和对照组各30例48髋。治疗组给予内服健脾补肾中药,1剂/d,外用活血化瘀中药,2~3次/d,体外冲击波每日1次;对照组只给予体外冲击波,1次/d。两组均治疗6个月。比较两组患者治疗前后Harris评分、低密度脂蛋白(LDL)、总胆固醇(TC)、血清骨钙素(BGP),并检测安全性指标。结果:两组患者治疗后Harris评分较治疗前均明显改善(P<0.05),治疗组治疗后Harris评分、LDL、TC、BGP均明显优于对照组(P<0.05)。治疗期间两组患者安全性指标未见异常。结论:健脾补肾中药内服活血化瘀中药外用联合体外冲击波能有效改善老年早期酒精性股骨头坏死患者临床症状,其作用机制可能与有效调节脂代谢及骨代谢相关。Objective:To observe the clinical curative effects and security of oral and external Chinese herbal medicine and extracorporeal shock wave for early elderly alcohol-induced osteonecrosis of the femoral head.Methods:Using prospective controlled study,all 60 cases of femur head necrosis accorded with the inclusive criteria were randomly divided into treatment group(30 cases,48 hips)and control group(30 cases,48 hips).The treatment group took traditional Chinese medicine Jianpi Bushen Formula orally one dose daily,and promoting blood circulation drugs for external use twice a day,and extracorporeal shock wave once a day.The control group took extracorporeal shock wave once a day.Two groups were treated for 6 months.We compared parameters like scores of Harris,low density lipoprotein(LDL),total cholesterol(TC),bone gla protein(BGP)and security index before and after treatment.Results:The patients in the two groups were all followed up for 6-12 months.After treatment,the treatment group's score of Harris,LDL,TC,BGP,IGF-1 were superior to those of the control group(P<0.05).No abnormal change of security index was found in both groups.Conclusion:Taking Chinese herbal medicine Jianpi Bushen Formula orally,and promoting blood circulation drugs for external use and extracorporeal shock wave may effectively improve clinical symptoms for early elderly alcohol-induced osteonecrosis of the femoral head.These effects may be related to effective regulation of metabolic balance of lipid and bone.
