中医护理在老年COPD稳定期患者延续护理中的应用

摘　　要：目的探讨中医护理在老年慢性阻塞性肺疾病(COPD)稳定期患者延续护理的应用价值。方法选取2017年1月—6月在老年科接受系统治疗后出院的COPD稳定期患者60例,随机分为观察组和对照组,各30例,观察组脱落1例。对照组采用常规出院指导,观察组在对照组的基础上实施中医护理团队主导的中医延续护理。分别于患者出院时、出院1、3、6个月评价COPD评估测试量表(CAT),记录患者半年内非计划再入院次数,调查延续护理满意度。结果观察组在出院3、6个月后的CAT评分均低于对照组(P <0. 05)。观察组出院后半年内非计划再入院次数为(0. 56±0. 23)次,低于对照组的(2. 85±0. 81)次(P <0. 05);观察组延续护理满意度高于对照组(P <0. 05)。结论延续性中医护理能提高老年COPD稳定期患者生活质量,降低患者非计划再住院次数,提高患者满意度,体现了中医护理的价值。Objective To investigate the application value of traditional Chinese medicine (TCM)nursing interventions in extended care of elderly patients with stable chronic obstructive pulmonary disease (COPD).Methods Totally 60 patients with stable COPD were randomly divided into the observation group (n=29 as 1 case lost to follow-up)and the control group(n=30).Patients in both two groups received routine care and health education before discharge from hospital,and those in the observation group were given TCM nursing interventions in extended care after discharge from the hospital.The COPD Assessment Test (CAT)was used to patients at discharge,1 month,3 months and 6 months after discharge.The unplanned readmission and satisfaction degree of extended care were assessed and compared between two groups.Results The CAT score in the observation group was lower than that in the control group at 3 months and 6 months after discharge (P<0.05).The observation group had a fewer times of unplanned readmission compared with that in the control group (0.56±0.23 vs.2.85±0.81,P<0.05).Patients in the observation group were more satisfied with the extended care service (P<0.05).Conclusion The extended TCM nursing interventions are effective to improve the quality of life in elderly patients with stable COPD,reduce the unplanned readmission and improve the satisfactory degree of patients.
