摘 要:目的探讨超声联合MR检查评估颈动脉斑块易损性的二维超声回声分级与脑白质病变发生的关系。方法对46例脑白质病变和54例非脑白质病变患者进行颈动脉超声和MR检查,依据MR检查结果,比较脑白质病变和非脑白质病变患者心血管风险因素和颈动脉斑块二维超声回声分级差异,分析脑白质病变与颈动脉斑块二维超声回声分级的关系,评估颈动脉斑块二维超声回声分级预测脑白质病变的价值。结果脑白质病变患者与非脑白质病变患者之间常规心血管风险因素无明显统计学意义,而颈动脉斑块二维超声回声分级差异明显,脑白质病变患者颈动脉斑块二维超声回声Ⅰ和Ⅱ级比例更高,Ⅲ级比例更低,脑白质病变发生与颈动脉斑块二维超声回声分级呈负相关。ROC分析显示,以颈动脉斑块二维超声回声分级≤Ⅱ级预测脑白质病变发生的敏感性和特异性分别为80. 43%,55. 56%。结论颈动脉斑块二维超声回声分级与MR检查诊断的脑白质病变密切相关。Objective To evaluate the relation of white matter lesions to the two-dimensional ultrasound classification grade of carotid plaque reflecting carotid plaque vulnerability by the combination of ultrasonography and MRI. Methods 46 patients with white matter lesions and 54 patients without white matter lesions were studied by ultrasonography and MRI. According to MRI, the differences of cardiovascular risk factors and the two-dimensional ultrasound classification grade of carotid plaque were compared between patients with white matter lesions and patients without white matter lesions, the relation of white matter lesions to the two-dimensional ultrasound classification grade of carotid plaque was analysed, and predictive utility of the two-dimensional ultrasound classification grade of carotid plaque for white matter lesions was evaluated. Results There are no differences of cardiovascular risk factors between patients with white matter lesions and patients without white matter lesions. And the differences of two-dimensional ultrasound classification grade of carotid plaque between patients with white matter lesions and patients without white matter lesions are significant, the ratio of I and II grade is higher and the ratio of III grade is lower in patients with white matter lesions. The two-dimensional ultrasound classification grade of carotid plaque is negatively correlated with white matter lesions. The sensitivity and specificity to prediction of white matter lesions by the two-dimensional ultrasound classification grade of carotid plaque (cutoff value≤II) is 80.43% and 55.56%, respectively. Conclusion The two-dimensional ultrasound classification grade of carotid plaque is closely related to white matter lesions diagnosed by MRI.
