摘 要:目的建立斑点追踪技术(speckle tracking imaging,STI)测量正常儿童三尖瓣环位移(tricuspid annular dis-placement,TAD)的回归方程。方法 0~18岁健康儿童182例,经胸二维超声心动图采集心尖四腔心切面图像,Qlab10. 0软件分析输出TAD各参数,包括:三尖瓣前瓣附着点位移(TAD1)、三尖瓣隔瓣附着点位移(TAD2)、瓣环连线中点位移(TADm)和右室长轴缩短分数(longitudinal shortening fraction,LSF)。分析TAD各参数的影响因素,以及TAD1与M型三尖瓣环收缩期位移(tricuspidannularplanesystolicexcursion,TAPSE)的一致性。结果 TAD各参数均不受性别影响(P> 0. 05),各参数与年龄相关性最高。对TAD各参数与年龄进行线性拟合〔TAD1:Y=14. 51+0. 42X(r=0. 746,P <0. 001),TAD2:Y=8. 79+0. 26X(r=0. 609,P <0. 001),TADm:Y=11. 81+0. 36X(r=0. 810,P <0. 001)〕,LSF:Y=26. 36-0. 46X(r=-0. 768,P <0. 001)。结论临床上获取儿童TAD各参数方法可行、简便。TAD各参数的回归方程已建立,有益于评估先天性心脏病或获得性心脏病儿童TAD情况及右心室收缩功能。Objective To establish the regression equations of tricuspid annular displacement (TAD) in healthy children by speckle tracking imaging (STI). Methods Two-dimensional transthoracic echocardiographic apical four-chamber views were obtained in 182 healthy children. The TAD parameters were output by the Qlab 10.0, including tricuspid annulus displacement of the insertion of the anterior leaflet (TAD1), of the septal leaflet (TAD2), the tricuspid annulus midpoint displacement (TADm) and the right ventricular longitudinal fractional shortening (LSF). The possible influence factors of TAD parameters were discussed and the consistency between TAD1 and M-mode tricuspid annular plane systolic excursion (TAPSE) were analysed. Results TAD parameters were not influenced by gender ( P >0.05). Their correlations with age were highest. The linear fitting of TAD parameters with age was established 〔TAD1:Y=14.51+0.42X( r =0.746, P <0.001), TAD2:Y=8.79+0.26X ( r =0.609, P <0.001), TADm: Y=11.81+0.36X ( r =0.810, P <0.001), LSF: Y=26.36-0.46X ( r =-0.768, P <0.001)〕. Conclusion TAD is feasible and easily obtained among children. Regression equations are provided and may help interpreting the TAD measurements and right ventricular function in children with congenital or acquired heart diseases.
