“立体定位”内眦赘皮矫正术的临床应用研究

摘　　要：目的:介绍一种"立体定位"内眦赘皮矫正术并总结其临床疗效。方法:切口设计为不对称"Z"形,术中内眦韧带深部固定形成新内眦点,内眦点处皮缘无张力对合,保证新建内眦具有正常的解剖结构和美观外形,形成内眦部鼻侧生理性凹陷,达到内眦部结构"立体定位"目的。结果:经随访,患者术后切口均一期愈合,118例均获随访,随访时间6个月～2年,平均8.6个月。1例患者术后早期内眦瘢痕较明显,其余患者内眦赘皮术后均改善良好,内眦间距明显缩窄,双侧睑裂长度增加,泪阜充分显露,内眦角形态自然,鼻侧生理性凹陷显现,术区未见明显瘢痕,效果满意。结论:"立体定位"内眦赘皮矫正术具有手术设计及操作简便,创伤小,术后形态自然、美观,效果明显、稳定、持久,瘢痕不明显等优点。Objective To introduce a stereotactic technique for correction of the inner canthus and to summarize its clinical efficacy.Methods The design of the incision is asymmetric"Z".The new inner canthus point was formed by deep fixation of the medial canthus ligament during the operation.The new inner canthus had normal anatomical structure and beautiful appearance,and formed physiological depression on the nasal side of the inner canthus,so as to achieve the purpose of stereo positioning of the inner canthus structure.Results All the incisions healed by first intention.All 118 patients were followed up for 6 months to 2 years,with an average of 8.6 months.In one case,the scar of the inner canthus was obvious in the early stage after operation,but in the other cases,the epicanthus was improved well,the distance between the inner canthus was obviously narrowed,the length of the bilateral eyelid fissure was increased,the tear caruncle was fully exposed,the shape of the inner canthus angle was natural,the nasal physiological depression appeared,and no obvious scar was found in the operation area.Conclusion Stereotactic correction of epicanthus has the advantages of simple design and operation,less trauma,natural and beautiful shape,obvious,stable and lasting effect,and no obvious scar.
