摘　　要：目的:研究分析在复合型髋臼骨折的手术治疗中采用不同手术入路的临床作用效果。方法:选取2013年1月-2016年12月在笔者所在医院进行手术治疗的70例复合型髋臼骨折患者,根据患者的不同骨折类型采用不同的手术入路方法,主要分为三组,A组采用髂腹股沟入路方法(IIA),B组采用改良Stoppa入路方法 (MSA),C组患者采用腹直肌外侧入路方法 (PRA),比对分析三组患者术中骨折显露情况、关节功能评分情况及术后并发症的发生情况。结果:B组和C组患者的术中骨折显露时间和术中显露时出血量与A组的相比明显较少;B组和C组患者在不同时间的关节功能评分与A组的相比明显较高;B组和C组患者的术后并发症的发生率明显低于A组患者,差异均有统计学意义(P<0.05)。结论:复合型髋臼骨折的手术治疗不能绝对化,要根据骨折的特点和患者自身选取最佳的手术方式,既能保证手术的顺利进行,也能保证患者的术后康复情况。Objective:To investigate the clinical effects of different surgical approaches in the surgical treatment of complex acetabular fractures.Method:From January 2013 to December 2016,70 cases of surgical treatment of complex acetabular fracture patients in our hospital,according to patients with different types of using different surgical approach,mainly divided into three groups,group A with ilioinguinal approach(IIA),group B used modified Stoppa approach(MSA),group C used Para-rectus approach(PRA),three groups of patients with fracture exposed,the score of joint function and postoperative complications were compared.Result:Group B and group C of patients with fracture exposure time and intraoperative bleeding were significantly less than those in the group A,the joint function score of group B and group C patients in different periods were significantly higher those in the group A,group B and group C of patients with postoperative complications were significantly less than that in the group A,and the differences were all statistically significant(P<0.05).Conclusion:The operative treatment of complex acetabular fractures can not be absolutization.The best way of operation is to choose the characteristics of the root fracture and the patients themselves,which can not only guarantee the smooth operation of the operation,but also ensure the postoperative rehabilitation of the patients.
