行PFNA手术治疗老年股骨转子间骨折的疗效分析

摘　　要：目的探究对于老年骨骼转子间骨折采取PFNA手术的治疗效果。方法采取该院2012年1月—2016年12月期间前来就诊的股骨转子间骨折老年患者70例作为研究对象进行研究。根据患者采取治疗方式的不同分为对照组和治疗组,其中对照组患者采取动力髋螺钉固定术,治疗组患者采取髓内固定系统(PFNA),对比两组患者的疗效。结果治疗组患者的术中出血量为(155.33±39.77)m L,手术时间为(83.54±47.28)min,下床负重活动时间为(10.38±2.73)d,均显著低于对照组患者(t=12.315,9.886,4.423,P<0.05);Harris评分为(84.56±11.28)分,显著高于对照组患者属于等级良(t=6.815,P<0.05);对照组患者并发症发生率为6/35(17.14%);观察组患者并发症发生率为2/35(5.71%),显著低于对照组患者(χ~2=6.743,P<0.05)。结论采取PFNA的方式治疗老年股骨转子间骨折风险小,有利于患者减少恢复时间从而尽早下床行走以及负重,并具有较小的并发症发生率,值得临床参考。Objective To study the curative effect of PFNA surgery in treatment of senile intertrochanteric fracture.Methods 70 cases of senile patients with intertrochanteric fracture treated in our hospital from January 2012 to December 2016 were selected,and divided into two groups according to different treatment methods,the control group used the dynamic hip screw fixation,while the treatment group used the PFNA,and the curative effect was compared between the two groups.Results The intraoperative bleeding amount,operation time,time to get out of bed in the treatment group were respectively(155.33±39.77)mL,(83.54±47.28)min,(10.38±2.73)d,which were obviously lower than those in the control group(t=12.315,9.886,4.423,P<0.05);Harris score was(84.56±11.28)points,obviously higher than that in the control group(good level)(t=6.815,P<0.05),and the incidence rate of complications in the observation group was obviously lower than that in the control group[2/35(5.71%)vs 6/35(17.14%)],(χ^2=6.743,P<0.05).Conclusion The PFNA surgery in treatment of senile intertrochanteric fracture is conductive to reducing the recovery time with lower incidence rate of complications,and it is worth clinical reference.
