摘  要:目的探讨小儿气道异物诊断中运用多层螺旋CT扫描及图像后处理技术的临床价值。方法选择2010年1月—2018年2月期间该院收治的65例小儿气道异物患儿为研究对象,均行X线和多层螺旋CT扫描,对其临床资料进行回顾性分析。结果所有患儿均顺利完成检查,Ⅰ型、Ⅱ型以及Ⅲ型异物的CT轴位+后处理、CT轴位以及X线诊断准确率分别为100.00%(5/5)、97.78%(44/45)、93.33%(14/15);100.00%(5/5)、93.33%(42/45)、73.33%(11/15);20.00%(1/5)、64.44%(29/45)、46.67%(7/15);同时,CT轴位+后处理的诊断准确率高于X线诊断和CT轴位检查(P<0.05)。经MPR分析,65例均出现异常密度影,其40例为等密度影,占61.54%,同时,在CTVB图像中,可见4例支气管或气管正常,占6.15%,36例支气管或气管出现不完全阻塞,占55.38%,25例支气管或气管出现完全阻塞,占38.46%。结论临床上给予小儿气道异物患儿多层螺旋CT扫描+图像后处理技术可以提高诊断准确率。Objective To investigate the clinical value of multi-slice spiral CT scanning and image post-processing in the diagnosis of airway foreign body in children. Methods 65 children with pediatric airway foreign bodies admitted to the hospital from January 2010 to February 2018 were enrolled in the study. X-ray and multi-slice spiral CT scans were performed. The clinical data were retrospec-tively analyzed. Results All patients underwent successful examination. The accuracy of CT axial position + post-processing, CT axial position and X-ray diagnosis of type I, type II and type III foreign bodies were 100.00%(5/5) and 97.78%(44/45, respectively),93.33%(14/15); 100.00%(5/5), 93.33%(42/45), 73.33%(11/15); 20.00%(1/5), 64.44%(29/45) 46.67%(7/15); at the same time, the diagnostic accuracy of CT axial position + post-processing was higher than that of X-ray diagnosis and CT axial examination( P<0.05). According to MPR analysis, abnormal density was observed in 65 cases, and 40 cases were equal density shadows, accounting for 61.54%. At the same time, in the CTVB images, 4 cases of bronchial or tracheal were normal, accounting for 6.15%, and 36 cases of bronchial or trachea did not appear complete obstruction, accounting for 55.38%, 25 cases of bronchial or tracheal tube completely blocked, accounting for 38.46%. Conclusion Multi-slice spiral CT scan + image post-processing technique for children with airway foreign body in clinic can improve the diagnostic accuracy.
