摘 要:目的探讨银杏叶提取物(EGb)对体外冲击波碎石术(ESWL)所致的早期肾损伤是否有保护作用。方法 80例行ESWL术的单侧肾结石病人按随机数字表法分成观察组和对照组,每组40例,观察组于ESWL术前0. 5 h开始,给予银杏叶提取物20 m L加入生理盐水250 m L静脉滴注,每12小时1次,给药5 d;对照组同时间节点给予生理盐水250 m L静脉滴注。两组分别收集ESWL术前2 h、ESWL术后2 h、6 h、12 h、24 h、1周尿液标本10 m L,测定尿液中的中性粒细胞明胶酶相关载脂蛋白(u NGAL)和尿胱抑素C(u Cys-C)水平。结果 u NGAL在两组中于ESWL术后2 h开始升高,12 h达到高峰,1周时恢复到术前水平。观察组u NGAL于ESWL术后12h为(17. 56±3. 94) ng/m L,低于对照组(22. 68±4. 82) ng/m L,其差异有统计学意义(t=3. 56,P=0. 025)。u Cys-C在两组中于ESWL术后6 h开始升高,12 h达到高峰,24 h有所下降,1周时仍高于术前水平。观察组u Cys-C于ESWL术后12 h达(35. 61±6. 24) ng/m L,低于对照组(41. 28±5. 57) ng/m L,其差异有统计学意义(t=4. 47,P=0. 017)。结论 EGb能有效防护ESWL所致的早期肾小球和肾小管的损伤,其机制尚需进一步研究。Objective To analyse the effect of extract of ginkgo biloba(EGb)on the protection of renal injury caused by the extracorporeal shock wave lithotripsy(ESWL).Methods The 80 patients with unilateral kidney stones underwent extracorporeal shock wave lithotripsy from January to December 2016 in the gongli hospital were randomly divided into observation group and control group according to random number table,with forty patients in each group.The observation group were treated with EGb,while the control group was administrated with 0.9%sodium chloride.The specimen of urine at ESWL preoperative 2 h and postoperative 2 h,6 h,12 h,24 h,1w were collected.Apply ELISA for testing the uNGAL and uCys-C levels of patients and analyzed the expression of different times.Results uNGAL began to increase in the two groups at 2h after ESWL,reached the peak at 12 h,and returned to preoperative level at 1 w.The observation group uNGAL was(17.56±3.94)ng/mL at 12h after ESWL,lower than that of the control group which was(22.68±4.82)ng/mL,the difference was statistically significant(t=3.56,P=0.025).uCys-C increased in the two groups at 6h after ESWL,reached the peak at 12 h,decreased at 24 h,and still higher than the preoperative level at 1w.The observation group uCys-C was(35.61±6.24)ng/mL at 12 h after ESWL,which was(41.28±5.57)ng/mL lower than that of the control group,the difference was statistically significant(t=4.47,P=0.017).Conclusions EGb can effectively protect the renal injury in the ESWL,and its mechanism still needs further study.
