咽鼓管吹张注药联合皮质类固醇激素治疗急性分泌性中耳炎的效果

摘　　要：目的探讨咽鼓管吹张注药联合皮质类固醇激素治疗急性分泌性中耳炎的效果。方法选取我院2017年9月至2018年4月收治的急性分泌性中耳炎患者60例作为研究对象,依据治疗方案不同将其分为对照组(n=30,单纯使用皮质类固醇激素治疗)和研究组(n=30,使用咽鼓管吹张注药联合皮质类固醇激素治疗)。随访3个月,比较两组患者的临床疗效。结果研究组治疗总有效率为90.00%,高于对照组的66.67%(P<0.05);治疗后,两组患者气导平均阈值均低于治疗前,且研究组低于对照组(P<0.05);研究组复发率为3.70%,略低于对照组的20.00%,但两组差异不显著(P>0.05)。两组患者治疗期间均无咽鼓管损伤出血、剧烈疼痛等不良反应;随访3个月,无咽鼓管狭窄、持续性眩晕等并发症。结论咽鼓管吹张注药联合皮质类固醇激素治疗急性分泌性中耳炎的临床效果确切,可有效纠正患者气导平均阈值,无严重不良反应,安全性高,且可预防复发,值得临床推广应用。Objective To investigate the effect of eustachian tube blowing injection combined with corticosteroid hormones in the treatment of acute secretory otitis media.Methods Sixty patients with acute secretory otitis media treated in our hospital from September 2017 to April 2018 were selected as the study objects.They were divided into control group (n=30, treated with corticosteroid hormones alone) and research group (n=30, treated with eustachian tube blowing injection combined with corticosteroid hormones) according to different treatment regimen.Follow-up for 3 months, the clinical effects of the two groups were compared.Results The total effective rate of treatment in the research group was 90.00%, which was higher than 66.67% in the control group(P<0.05).After treatment, the average threshold of air conduction in both groups were lower than those before treatment, and that in the research group was slightly lower than the control group(P<0.05).The recurrence rate in the research group was 3.70%, which was slightly lower than 20.00% in the control group, but there was no significant difference between the two groups(P>0.05).There were no adverse reactions such as bleeding and severe pain during treatment in both groups, and no complications such as stenosis and persistent vertigo of eustachian tube were observed after 3 months follow-up.Conclusion Eustachian tube blowing injection combined with corticosteroid hormones in the treatment of acute secretory otitis media has a definite clinical effect.It can effectively correct the average threshold of patients' air conduction, without serious adverse reactions.It has high safety and can prevent recurrence.It is worthy of clinical popularization and application.
