无创阻抗心动图评价大株红景天注射液治疗高龄老年慢性心力衰竭的疗效研究

摘　　要：目的利用无创阻抗心动图(ICG)评价大株红景天注射液治疗高龄老年慢性心力衰竭(CHF)的疗效。方法将90例高龄CHF患者按照随机数字表法分为2组,对照组45例予西医常规抗心力衰竭治疗,治疗组45例在对照组治疗基础上加大株红景天注射液静脉滴注治疗,2组均治疗14 d后统计疗效。比较2组治疗前后血清N末端脑钠肽前体(NT-pro BNP)水平、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室射血分数(LVEF)及ICG心率(HR)、心脏指数(CI)、体循环外周血管阻力指数(SVRI)和胸液体容积(TFC)变化情况。结果治疗组总有效率(86. 7%)高于对照组(77. 8%,P <0. 05)。2组治疗后NT-pro BNP水平均较本组治疗前降低(P <0. 05),LVEDD及LVESD缩小(P <0. 05),LVEF增加(P <0. 05),且2组组间比较差异均有统计学意义(P <0. 05)。2组治疗后HR、SVRI、TFC均较本组治疗前下降(P <0. 05),CI升高(P <0. 05),且2组组间比较差异均有统计学意义(P <0. 05)。结论 ICG显示,大株红景天注射液可明显改善高龄老年CHF患者血流动力学异常,提高心功能,临床疗效确切,安全性良好。Objective The non-invasive impedance cardiogram(ICG)was used to evaluate the efficacy of Sofren injection in the treatment of elderly patients with chronic heart failure(CHF).Methods 90 advanced patients with CHF were divided into two groups according to the random number table method.45 cases in control group were treated by western medicine for anti-heart failure.45 cases in treatment group were treated by Sofren injection on the basis of control group.The curative effect was measured after 14 days of treatment in both groups.The changes of serum N-terminal-pro-brain natriuretic peptide(NT-proBNP),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(LVEF),heart rate(HR),cardiac index(CI),systemic peripheral vascular resistance index(SVRI)and pleural fluid volume(TFC)before and after treatment were compared between the two groups.Results The total effective rate of the treatment group(86.7%)was higher than that of the control group(77.8%,P<0.05).The levels of NT-proBNP in the two groups were lower than those in the group before treatment(P<0.05),LVEDD and LVESD were decreased(P<0.05),LVEF was increased(P<0.05),and the difference between the two groups was statistically significant(P<0.05).The HR,SVRI and TFC of the two groups were lower than those in the group before treatment(P<0.05)and CI increased(P<0.05),and the difference between the two groups was statistically significant(P<0.05).Conclusion The ICG index shows that Sofren injection can significantly improve the cardiac function of elderly patients with CHF.The clinical curative effect is clear and the safety is good.
