全螺纹加压螺钉联合微型接骨板治疗单纯距舟关节炎融合中的效果

摘　　要：目的探讨全螺纹加压螺钉联合微型接骨板在单纯距舟关节融合中的疗效。方法对2015年5月至2017年11月在我院采用全螺纹加压螺钉联合微型接骨板治疗的15例距舟骨性关节炎患者行回顾性分析。其中男10例(10足),女5例(5足)。记录骨性融合时间,对比术前术后AOFAS、VAS评分。结果 15例患者(15足)术后随访全部骨性融合,骨性融合平均时间10.5周(8～13周)。平均AOFAS评分从术前的(30.7±7.2)分(19～42分)提高致术后的(85.3±7.1)分(55～97分);平均VAS评分由术前的(8.5±1.0)分(4～10分)改善为术后的(1.5±1.2)分(0～5分),手术前后AOFAS、VAS评分差异有统计学意义。结论全螺纹加压螺钉联合微型接骨板内固定可以为距舟关节融合提供三维坚强内固定,允许患者早期活动,提高骨性融合率,并在短期、中期随访中取得了满意的疗效。Objective To investigate the effect of full-thread compression screw combined with mini-plates in isolated talonavicular fusion. Methods A total of 15 patients with talus scaphoid osteoarthritis treated with total screw compression combined with mini-plate in our hospital from May 2015 to November 2017 were retrospectively analyzed. There are 10 males (10 feet) and 5 females (5 feet). The time of complete osseous fusion was recorded and the AOFAS and VAS scores were compared before and after surgery. Results Fifteen patients (15 feet) were followed up for complete osseous fusion. The average time of complete osseous fusion was 10.5 weeks (8-13 weeks). AOFAS score was improved from preoperative average (30.7±7.2)(19-42 points) to postoperative average (85.3±7.1)(55-97 points); VAS score was reduced from preoperative average (8.5±1.0)(4-10 points) to postoperative average (1.5±1.2)(0-5 points), which had statistical significances. Conclusion Full-threaded compression screw combined with mini-plate internal fixation can provide three-dimensional rigid internal fixation for talus-scaphoid joint fusion, allow patients to move early, improve the rate of bone fusion, and achieve satisfactory results in short-term and medium-term follow-up.
