超声引导下髂腹下、髂腹股沟神经、腹横肌平面及生殖股神经联合阻滞对疝气修补术有效性及安全性评价

摘　　要：目的:探讨超声引导下髂腹股沟神经、髂腹下神经、腹横肌平面及生殖股神经联合阻滞用于腹股沟疝手术的安全性及有效性。方法:选取就诊的96例疝气患者按麻醉方式的不同随机分为两组,一组为超声引导下髂腹股沟神经、髂腹下神经、腹横肌平面及生殖股神经联合阻滞组(观察组);对照组患者进行插管全身麻醉,每组48例。比较两组应用效果,即两组患者的麻醉后不良反应(躁动现象和切皮反应)、血压及心率变化情况和对比结果。结果:观察组在手术后出现躁动现象的比率占6. 5%,而对照组躁动平均发生率高达19. 3%,差异有统计学意义(P <0. 05);术前两组患者的血压和心率均无统计学差异(P>0. 05),术中切皮和术后苏醒以上各指标差异明显,有统计学意义(P <0. 05)。结论:与全身麻醉相比,超声引导下髂腹股沟神经、髂腹下神经、腹横肌平面及生殖股神经联合阻滞应用于腹股沟疝手术安全、可靠。Objective to investigate the safety and efficacy of ultrasound-guided combined treatment of ilio-inguinal nerve,ilio-abdominal nerve,transverse abdominal plane and genital nerve block for inguinal hernia.Method 96 cases of hernia treated in our hospital were randomly divided into two groups according to different anesthesia methods.One group was ultrasound-guided iliovaginal nerve,ilio-abdominal nerve,transverse abdominal plane and reproductive Femoral nerve block group(observation group);control group patients underwent intubation general anesthesia,48 cases in each group.The effects of two groups were compared,that is,the adverse reactions after anesthesia(the incidence of agitation),the changes of blood pressure and heart rate and the comparison results between the two groups.Results The rate of restlessness in the observation group after operation was 6.5%,while the average incidence of agitation in the control group was as high as 19.3%(P<0.05).There was no statistical difference in blood pressure,oxygen saturation and heart rate between the two groups(P>0.05).There was significant difference between the operation of incision and postoperative recovery(P<0.05).Conclusion Compared with general anesthesia,the application of ultrasound-guided ilio-inguinal nerve,ilio-abdominal nerve,transverse abdominal plane and genital nerve block in the inguinal hernia operation is safe and reliable.
