集束化护理在极低出生体重早产儿母乳喂养中的应用

摘　　要：目的探讨集束化护理在极低出生体重早产儿母乳喂养中应用的效果。方法将2015年5月～2016年5月住院的93例极低出生体重早产儿作为对照组,实施日常护理及入院时母乳喂养健康宣教;将2016年6月～2017年6月住院的107例极低出生体重早产儿作为观察组,在日常护理及入院时母乳喂养健康宣基础上实施母乳集束化护理,比较两组母乳喂养率及喂养不耐受、新生儿坏死性小肠结肠炎、感染等并发症的发生率。结果观察组纯母乳喂养率与母乳喂养+配方奶喂养率均明显高于对照组,差异有统计学意义(P<005),但观察组喂养不耐受、新生儿坏死性小肠结肠炎、感染发生率均低于对照组,差异有统计学意义(P<005)。结论对极低出生体重早产儿实施母乳喂养的集束化护理,可以促使母乳喂养科学化、体系化、标准化,确保安全母乳喂养,提高母乳喂养率,降低喂养不耐受、新生儿坏死性小肠结肠炎、感染等发生率,促进早产儿的健康发育。Objective To investigate the effect of cluster nursing on breastfeeding of very low birth weight preterm infants. Methods A total of 93 cases of very low birth weight preterm infants hospitalized from May 2015 to May 2016 were selected as the control group, who received daily nursing and breastfeeding health education on admission. 107 cases of very low birth weight preterm children hospitalized from June 2016 to June 2017 were selected as observation group, who were treated with breastfeeding cluster nursing based on the daily nursing and breast-feeding health declaration on admission. The rate of breastfeeding and the incidence of complications including feeding intolerance, neonatal necrotizing enterocolitis, infections between two groups were compared. Results The rates of exclusive breastfeeding and breastfeeding+formula feeding in observation group were significantly higher than those in control group, and the difference was significant(P<0.05), but the incidence of feeding intolerance, neonatal necrotizing enterocolitis and infection in observation group was lower than that of the control group, and the difference was statistically significant(P<0.05). Conclusion The cluster nursing of breastfeeding preterm infants with very low birth weight can promote scientific, systematic and standardized breastfeeding, ensure safe breastfeeding, improve breastfeeding rate, reduce the rates of feeding intolerance, neonatal necrotizing enterocolitis, infection, etc, to promote the healthy development of premature children.
