摘　　要：目的比较经股动脉顺行入路和经桡动脉逆行入路介入治疗左锁骨下动脉闭塞的有效性和安全性。方法回顾性分析2014年1月～2017年12月我院收治的72例左锁骨下动脉闭塞患者血管腔内治疗的效果,其中36例经股动脉顺行入路行介入治疗(股动脉组),36例经桡动脉逆行入路行介入治疗(桡动脉组)。结果股动脉组手术成功率、并发症发生率分别为83. 33%和16. 67%,桡动脉组分别为97. 22%和2. 78%。两组比较,桡动脉组手术成功率高于股动脉组,而手术并发症发生率显著低于股动脉组,差异均有统计学意义(P <0. 05)。结论经桡动脉逆行入路介入治疗左锁骨下动脉闭塞动脉开通效果好,并发症少,安全性高。Objective To compare the efficacy and safety of interventional treatment of left subclavian artery occlusion by transfemoral artery anterograde approach and transradial artery retrograde approach.Methods A total of 72 patients with left subclavian artery occlusion admitted to our hospital from January2014 to December 2017 were retrospectively analyzed. 36 patients underwent transfemoral artery anterograde interventional therapy( as femoral artery group) and 36 patients underwent transradial artery retrograde interventional therapy( as radial artery group). The success rate and complications rate were compared between the two groups. Results The success rate and complication rate was 83. 33% and 16. 67% in femoral artery group,and 97. 22% and 2. 78% in radial artery group,respectively. The success rate of the operation was higher and the incidence of complication was lower in the radial artery group than those in the femoral artery group,and the differences were significant statistically( P < 0. 05). Conclusion Transradial artery retrograde interventional therapy can achieve an reliable effect with preferable arterial recanalization rate,few complication and high safety for left subclavian artery occlusion.
