锁骨钩钢板联合双股不可吸收线治疗新鲜Tossy Ⅲ型肩锁关节脱位

摘　　要：目的探讨锁骨钩钢板联合双股不可吸收线治疗新鲜TossyⅢ型肩锁关节脱位的临床疗效。方法将74例新鲜TossyⅢ型肩锁关节脱位患者随机分为观察组和对照组,每组37例。对照组采用锁骨钩钢板固定,观察组采用锁骨钩钢板联合双股不可吸收线固定。术后12个月评定两组肩关节活动度、疼痛VAS评分、肩关节功能优良率及并发症发生率。结果患者均获得12个月的随访。术后12个月,肩关节活动度观察组显著高于对照组(P <0. 01),疼痛VAS评分观察组显著低于对照组(P <0. 01);参照Karlsson标准评价疗效,肩关节功能优良率观察组高于对照组(97. 30%vs 75. 68%,P <0. 05)。观察组并发症发生率低于对照组,但差异无统计学意义(P> 0. 05)。结论锁骨钩钢板联合双股不可吸收线治疗新鲜TossyⅢ型肩锁关节脱位能够降低患肩疼痛,提高肩关节功能,且不增加并发症发生率,临床疗效满意。ObjectiveTo investigate the therapeutic effect of clavicular hook plate combined double non-absorbable suture in the treatment of fresh Tossy typeⅢacromioclavicular joint dislocation.MethodsThe 74 patients with Tossy typeⅢacromioclavicular joint dislocation were evenly divided into the observation group and the control group,37 cases in each group.Patients in the control group were treated with clavicular hook plate,while patients in the observation group were treated with clavicular hook plate combined double non-absorbable sutures.At 12 months after the surgery,the mobility of shoulder joint,the VAS,the excellent-good rate of shoulder joint function and the incidence of complications were evaluated and compared between the two groups.Results All patients were followed up for 12 months.At 12 months after operation,the mobility of shoulder joint of the patients in the observation group was significantly higher than that in the control group(P<0.01),and the VAS was significantly lower than that in the control group(P<0.01).According to the Karlsson standard,the excellent-good rate of shoulder joint function in the observation group was higher than that in the control group(97.30%vs 75.68%,P<0.05).The incidence rate of postoperative complications in the observation group was lower than the control group,but the difference did not reach statistical significance(P<0.05).Conclusions Clavicular hook plate combined double non-absorbable sutures in the treatment of fresh Tossy typeⅢacromioclavicular joint dislocation can reduce the patient′s shoulder pain,improve shoulder joint function,and do not increase the incidence of complications,which means the clinical effect is satisfactory.
