摘 要:目的观察氢及饱和氢盐水对心肌缺血再灌注损伤家兔心肌梗死面积和血流动力学的影响。方法随机将40只家兔分为4组,每组10只。正常组开胸后冠状动脉穿线不结扎;模型组开胸并结扎冠状动脉前降支60min,松解后静脉泵入生理盐水10mL/kg或腹腔空气10mL/kg;静脉氢盐水组给予静脉氢盐水10mL/kg,10min内注入;腹腔氢气组结扎冠状动脉前降支60min,松解后给予腹腔内氢气10mL/kg,10min内注入。应用依文氏兰和氯化三苯基四氮唑染色确定心肌梗死面积并分阶段测定左室收缩压(LVSP)、左室舒张末压(LVEDP)和正负等容收缩期左室压上升或下降速率(±dp/dt max)等。结果静脉氢盐水组和腹腔氢气组心肌梗死面积均明显小于模型组(P <0.01);静脉氢盐水组和腹腔氢气组心脏血流动力学指标LVSP、LVEDP和±dp/dt max均优于模型组(P <0.01)。结论静脉氢盐水和腹腔氢气均能减少缺血再灌注损伤心肌梗死面积,同时具有较好的血流动力学效应。Objective To explore the influence of hydrogen on the myocardial infarction area and hemodynamic in rabbits with myocardialischemia reperfusion injury.Methods Forty New Zealand rabbits were randomly divided into normal group(open heart coronary arterythread ligation,n =10),model group(thoracotomy and the anterior descending coronary artery ligation of vein pump after 60 minutesrelease into saline 10 mL/kg,n =5,or intraperitoneal air 10 mL/kg,n =5),intravenous hydrogen saline group,and abdominal hydrogengroup(the anterior descending coronary artery ligation respectively after 60 minutes hydrogen was given intravenous saline or intraperito-neal hydrogen 10 mL/kg,injection within 10 minutes,n =10).The myocardial infarction area was determined by Evans blue and TTCstaining.The left ventricular systolic blood pressure(LVSP),left ventricular final diastolic blood pressure(LVEDP),and the rate of in-crease or decrease(±dp/dt max)were measured in stages.Results Compared with the model group,the myocardial infarction area of theintravenous hydroaline group and the abdominal hydrogen group was significantly lower than that of the model group(P <0.01).Cardiachemodynamic indexes LVSP,LVEDP,and±dp/dt max in the intravenous hydrosaline group and the peritoneal hydrogen group were bet-ter than those in the model group(P <0.01).Conclusion Both intravenous hydrosaline and intraperitoneal hydrogen can reduce the my-ocardial infarction area after ischemia reperfusion injury and have good hemodynamic effect.
