血浆置换联合凉血化瘀方治疗慢性重型肝炎的临床观察

摘　　要：目的观察血浆置换(PE)联合中医凉血化瘀方治疗重型肝炎的临床疗效。方法选择本院血浆置换联合凉血化瘀方治疗慢性重型肝炎热毒血瘀证患者43例为观察组,40例仅接受血浆置换以及基础治疗的患者为对照组。治疗8周后,观察2组患者肝功能指标、凝血指标、T细胞亚群等变化。结果观察组有效率为81.39%,对照组为65.0%,有统计学差异(P<0.05)。2组患者经治疗后临床症状、中医证候积分、肝功能指标、凝血指标以及T细胞亚群均明显改善(P<0.01),观察组中医证候积分、肝功能指标、凝血指标、CD4+百分比以及CD4+/CD8+改善,明显优于对照组(P<0.05~0.01)。结论血浆置换联合凉血化瘀方可有效治疗重型肝炎,改善患者肝功能、凝血功能以及T细胞亚群。OBJECTIVE To observe the clinical efficacy of plasma exchange combined with Liangxue Huayu Formula on treating severe hepatitis.METHODS Patients diagnosed with severe hepatitis in observation group(43 cases)were treated with plasma exchange and Liangxue Huayu Formula.While the control group(40 cases)were only received plasma exchange and basic treatment.The liver function index,coagulation index and the change of T cell subsets the two groups were recorded after eight weeks of treatment.RESULTS The effective rate of the observation group was 81.39% while that of the control group was 65%.There was a statistical difference between the two groups(P<0.05).The clinical symptoms,TCM syndrome score and liver function index,coagulation index,and changes of T cell subsets were significantly improved after treatment(P<0.05).The improvement of TCM syndrome score,indicators of liver function and coagulation,the percentage of CD4+and CD4+/CD8+in observation group were significantly better than that of the control group(P<0.05,P<0.01).CONCLUSION Plasma exchange combined with Liangxue Huayu Formula can effectively treat severe hepatitis,as well as improving patients’ liver function,coagulative function and T cell subsets.
