摘  要:针对传统血压计袖带使用给患者带来的不便以及现有的利用单一脉搏波传导时间(pulse wave translation time,PWTT)进行无创血压监测稳定性较差等问题,本研究提出基于心电信号(electrocardiograph,ECG)-电容积脉搏波信号(photoplethysmo graphy,PPG)多特征参数融合的无创血压监测、检测方法,并设计相应的血压监测系统。系统通过采集ECG和PPG提取特征并建立血压计算模型。模型以PPG传导时间PWTT、PPG上升支时间与PPG周期之比λ和PPG峰峰值φ为自变量,实测血压值为因变量进行回归分析,得到收缩压(systolic blood pressure,SBP)和舒张压(diastolic blood pressure,DBP)回归方程,实现无袖带血压连续监测、检测。利用该方法对30名志愿者进行验证实验,结果显示,本系统所测SBP与实测SBP绝对误差均值E_d=2. 3067 mmHg,标准差S_D=1. 4633 mmHg; DBP绝对误差均值E_d=2. 4133 mmHg,标准差S_d=1. 9894 mmHg,符合AAMI推荐标准。通过Bland-Altman一致性分析,本系统与水银血压计所测SBP、DBP相关系数分别为0. 9878、0. 9730,一致性界限分别为(-4. 9300,5. 6770)、(-6. 1950,6. 0620)。实验结果证明,通过本方法可以实现无袖带血压的连续监测、检测,为便携式可穿戴设备血压实时监测及临床无袖带血压检测提供了可参考的实施方法。In view of the inconvenience caused by the use of the traditional sphygmomanometer cuffs and the poor stability of the existing continuous non-invasive blood pressure monitoring system by using the single parameter“pulse wave translation time(PWTT)”,a non-invasive blood pressure monitoring and detection method using the ECG-PPG multi-characteristic parameters were proposed and a corresponding blood pressure monitoring system was designed.The system extracted parameters from the collected electrocardiograms(ECG)and photoplethysmography(PPG)signals and established a blood pressure calculation model which used PPG translation time PWTT,ratioλof PPG rise time and PPG cycle,PPG peak-to-peak value as independent variables,and measured blood pressure values as the dependent variable for regression analysis to obtain the systolic blood pressure(SBP)and diastolic blood pressure(DBP)calculation equations to achieve continuous blood pressure monitoring and detection without cuff.Using this method,30 volunteers were tested and the results showed that for SBP,E d=2.3067 mmHg and S a=1.4633 mmHg,while for DBP E d=2.4133 mmHg and S a1.9894 mmHg,which were in line with AAMI recommendations.The Bland-Altman consistency analysis showed that the correlation coefficients of SBP and DBP measured by the system and mercury blood meter were 0.9878 and 0.9730,respectively,and the consistency limits were(-4.9300,5.6770)and(-6.1950,6.0620).The experimental results show that the continuous blood pressure monitoring and detection without cuff can be achieved by this method,and this method also provides a possible implementation for blood pressure real-time monitoring in portable wearable devices and sleeveless blood pressure detection for clinical.
