摘 要:目的:探析妊娠期糖尿病与妊娠期显性糖尿病孕妇临床特点的差异。方法:收集石河子大学医学院第一附属医院内分泌科自2017年10月至2018年5月初诊的妊娠高血糖孕妇,根据国际妊娠合并糖尿病共识小组(IADPSG)诊断标准,纳入妊娠期显性糖尿病(overt diabetes mellitus,ODM)组孕妇34例,妊娠期糖尿病(gestational diabetes mellitus,GDM)组孕妇32例,收集研究对象的基线资料,记录各项检测指标数据,比较分析两组孕妇临床特点的差异。结果:与GDM组孕妇相比,ODM组孕妇有更高的糖尿病家族史(P<0.05);两组间空腹血糖、餐后1h血糖、餐后2h血糖、糖化血红蛋白比较,ODM组显著高于GDM组(P<0.05);ODM组启用胰岛素降糖治疗的孕妇占88.24%(30/34),明显高于GDM组34.38%(11/32)(P<0.05)。两组间尿酮体阳性率、尿微量白蛋白异常率、尿微量白蛋白肌酐比值异常率比较差异无统计学意义(P>0.05)。结论:妊娠期显性糖尿病较妊娠期糖尿病有更明显的糖尿病家族史,有更高的空腹血糖、餐后1h血糖、餐后2h血糖、糖化血红蛋白,需要使用胰岛素治疗人数更多。Objective:To compare the clinical characteristics of pregnant women with gestational diabetes mellitus and overt diabetes mellitus.Methods:Pregnant patients who were newly diagnosed as diabetes by department of endocrinology of the first affiliated hospital of shihezi university School of Medicine during October 2017 to May 2018 were collected.According to IADPSG criteria,patients were diagnosed as gestational diabetes mellitus(GDM group,n=32)and overt diabetes mellitus(ODM group,n=34).We collected baseline information and lab results of subjects.Then compare the clinical characteristics between ODM group and GDM group.Results:compared with GDM group,ODM group with a higher family history of diabetes(P<0.05).The ODM group was significantly higher than the GDM group in fasting blood glucose,1h postprandial blood glucose,2h postprandial blood glucose and glycosylated hemoglobin.In the ODM group,88.24%(30/34)of pregnant patients were treated with insulin,which were significantly higher than 34.38%(11/32)in the GDM group(P<0.05).There was no statistically significant difference between the two groups in the positive rate of urinary ketone body,abnormal rate of urinary microalbumin,and abnormal rate of urinary microalbumin creatinine ratio(P>0.05).Conclusion:ODM has a more significant family history of diabetes than GDM,with higher fasting blood glucose,1h postprandial blood glucose,2h postprandial blood glucose,and glycosylated hemoglobin,and more patients with insulin treatment.
