社区2型糖尿病足溃疡相关因素的研究

摘　　要：目的:探讨社区2型糖尿病足溃疡的相关危险因素。方法:选取石河子社区2型糖尿病(T2DM)并且无足溃疡的患者242例,根据震动感觉阈值(VPT)分为无风险组(VPT≤10V),轻度风险组(10V<VPT≤15V),中度风险组(15V<VPT≤25V),重度风险组(VPT>25V),记录各组临床资料、测定生化指标。结果:各组之间吸烟行为、饮酒行为、体重指数、腰臀比、收缩压、舒张压、糖化血红蛋白、空腹血糖、餐后血糖、胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白之间差异无统计学意义(P>0.05),各组之间性别、年龄、病程差异有统计学意义(P<0.05),高度风险组比中度风险组、轻度风险组、无风险组年龄更大、病程更长,且男性患者比例增长,VPT与年龄、糖尿病病程、糖化血红蛋白、空腹血糖、餐后血糖水平呈正相关(P<0.05)。结论:2型糖尿病患者发生足溃疡风险与年龄、糖尿病病程、糖化血红蛋白、空腹血糖、餐后血糖有关。Objective:To explore the related risk factors of community type 2 diabetic foot ulcer.Methods:Select type 2 diabetes mellitus patient with no foot ulcer 242 cases,according to the vibration perception threshold(VPT),they were divided into four groups:no risk group(VPT≤10V),mild risk group(10V<VPT≤15V),moderate risk group(15V<VPT≤25V),severe risk group(VPT>25V),Collect clinical data of each group and detect biochemical indicators.Results:Smoking behavior、drinking behavior、body mass index、waist-hip ratio、systolic pressure、diastolic pressure、glycosylated hemoglobin、fasting blood-glucose、postprandial glucose、cholesterol、triglyceride、high-density liprotein、low-density liprotein had no significant difference among groups(P>0.05),the sex、age、diabetic course had significant difference among groups(P<0.05),the age of severe risk group was more older than moderate risk group、mild risk group and no risk group,the diabetic course of was more longer than moderate risk group、mild risk group and no risk group,the proportion of male patients of severe risk group was more greater than moderate risk group、mild risk group and no risk group,Analysis showed that vibration perception threshold was positively correlated with age diabetic course、glycosylated hemoglobin、fasting blood-glucose、postprandial correlated with age diabetic course、glycosylated hemoglobin、fasting blood-glucose、postprandial glucose(P<0.05).Conclusion:These finds suggested that the cause and development of type 2 diabetic foot ulcer was associated with course、age、glycosylated hemoglobin、fasting blood-glucose and postprandial glucose.
