李世增教授临证应用三仁汤经验探析

摘　　要：三仁汤出自《温病条辨》,具有宣畅气机,清热利湿的功效。李世增教授在临床上善用三仁汤治疗咳嗽、浸淫疮、头痛、淋证、虚劳、水肿等多种疾病,取得了良好的临床疗效。文章首先介绍了三仁汤的出处和现代应用,然后阐述了李世增教授在临证使用三仁汤的经验:谨守病机,异病同治;三焦并重;祛湿为主,清热为辅;遵循治则,用药灵活。最后通过列举和分析临床上的两则典型医案,向读者展示李世增教授在临床实践中辨证论治的思路,以期为同道提供参考。Sanren decoction,in Treatise on Differentiation and Treatment of Epidemic Febrile Diseases,has the effects of promoting qi activity,clearing heat and removing damp. Prof. Li Shizeng prefers this formula in the treatment of cough,eczema,headache,straguria,consumptive disease,edema,etc. The satisfactory therapeutic effects have been achieved. In the paper,the source and modern application of sanren decoction was introduced at first. Prof. Li’ s experiences in the application of this formula were explained. Strictly focusing on the pathogenesis,the different diseases were treated with the same method. The conditions of the upper,middle and lower jiao were equally important. Removing damp was the primary and the clearing heat was the assistant in treatment. The medication was prescribed flexibly in compliance with the treating principle. Finally,two typical cases were described in analysis. The thoughts of Prof. Li on the syndrome differentiation and treatment determination in clinical practice were introduced so as to provide the reference to the medical doctors.
