冠状动脉旁路移植术不同术式的临床对比分析

摘　　要：目的对比分析非体外循环冠状动脉旁路移植术(off-pump CABG)和体外循环冠状动脉旁路移植术(on-pump CABG)两种术式治疗冠心病的临床效果和中、远期疗效。方法回顾性分析383例冠状动脉旁路移植手术患者的资料,分别从两组中随机选取15例患者行冠状动脉造影(CAG)检查,观察移植血管通畅情况并对比分析疗效。结果 off-pump CABG组手术时间、辅助呼吸时间、ICU停留时间、术后住院时间短于on-pump CABG组,术中输血量少于on-pump CABG组,差异有统计学意义(P <0. 05)。两组乳内动脉桥、大隐静脉桥通畅率比较,差异无统计学意义(P> 0. 05)。结论 off-pump CABG治疗冠心病安全可行,随访结果显示两种术式临床效果相似。Objective To compare the clinical applied effective and long-term clinical outcomes of off-pump coronary artery bypass graft(off-pump CABG)and on-pump coronary artery bypass graft(on-pump CABG)in treating patients with coronary heart disease.Methods The data of 383 patients who underwent CABG procedures were reviewed,and 15 patients were randomly selected from two groups with coronary angiography to observe the patency rate and to compare the curative effect.Results The assisted respiratory time,the intensive care unit time,the hospitalization time of off-pump CABG group were significantly shorter than those of on-pump CABG,and blood transfusion volume of off-pump CABG group were significantly less than that of on-pump CABG group(P<0.05).There were no statistically significant differences in the patency rate of internal mammary artery and saphenous vein grafts between the two groups(P>0.05).Conclusion off-pump CABG is a clinically safe procedure to treat the patients of coronary heart disease,and follow-up results show that the clinical effects of two procedures are similar.
