摘 要:目的:对比分析零切迹颈前路椎间融合系统与传统钉板系统治疗脊髓型颈椎病的效果。方法:选取2015年3月-2017年5月来本院就诊的脊髓型颈椎病患者82例作为研究对象,按照随机数字表法将其分为对照组和观察组,每组41例。两组均行前路椎间减压融合术,对照组应用传统钉板系统,观察组应用零切迹颈前路椎间融合系统,对比两组的日本骨科协会评估分数(JOA评分)、手术指标、术后吞咽困难发生情况。结果:手术后,两组JOA评分均明显高于手术前,且观察组明显高于对照组,差异均有统计学意义(P<0.05)。两组手术时间和术中出血量对比,差异均无统计学意义(P>0.05)。观察组术后吞咽困难发生率为9.76%明显低于对照组的29.27%,差异有统计学意义(字2=4.970,P<0.05)。结论:与传统钉板系统相比,零切迹颈前路椎间融合系统治疗脊髓型颈椎病的效果更好,可以显著提高患者JOA评分,并降低吞咽困难的术后并发症,可进一步推广应用。Objective:To compare and analyze the effect of zero-incision cervical anterior interbody fusion system and traditional nail plate system in the treatment of cervical spondylotic myelopathy.Method:A total of 82 patients with cervical spondylotic myelopathy treated in our hospital from March 2015 to May 2017 were selected as the study objects.According to the random number table method,they were divided into control group and observation group,41 cases in each group.The anterior interbody fusion were performed in two groups,the control group was used traditional nail plate system,the observation group was used zero-incision cervical anterior interbody fusion system.The Japanese orthopaedic association assessment score(JOA score),surgical index and postoperative dysphagia between the two groups were compared.Result:After operation,the JOA scores of the two groups were significantly higher than those before operation,and the observation group was significantly higher than that of the control group,the differences were statistically significant(P<0.05).There were no significant differences in operation time and intraoperative blood volume between the two groups(P>0.05).The incidence of postoperative dysphagia in the observation group was 9.76%,which was significantly lower than 29.27%in the control group,the difference was statistically significant(χ^2=4.970,P<0.05).Conclusion:Compared with the traditional nail plate system,the zero-incision cervical anterior interbody fusion system is more effective in the treatment of cervical spondylotic myelopathy,which can significantly improve the JOA score of patients and reduce the postoperative complications of dysphagia,it can be further promoted and applied.
