栀黄止痛散外敷对急性痛风性关节炎患者血清IL-1β、TNF-α水平的影响

摘　　要：目的:观察栀黄止痛散外敷治疗急性痛风性关节炎(AGA)(湿热蕴结证)的临床疗效及对血清IL-1β、TNF-α水平的影响。方法:将60例AGA患者随机分为治疗组、对照组,每组各30例。对照组服用依托考昔片治疗,治疗组在对照组治疗的基础上联合应用栀黄止痛散调和外敷。观察两组患者治疗前及治疗7天后VAS疼痛评分、关节肿胀评分、血尿酸(UA)、血沉(ESR)、C反应蛋白(CRP)、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α),并做疗效评价。结果:用药7天后,治疗组总有效率为96.67%,高于对照组的86.67%,差异有统计学意义(Z> 1.96,P <0.05);治疗组疼痛、肿胀评分低于对照组,差异有统计学意义(P <0.05);治疗组血UA、CRP、IL-1β、TNF-α指标均低于对照组,差异有统计学意义(P <0.05);治疗组的ESR与对照组比较差异无统计学意义(P> 0.05)。结论:栀黄止痛散对AGA有显著疗效,可明显降低IL-1β、TNF-α等炎性因子水平。Objective:To observe the clinical efficacy of Zhihuang Zhitong San(栀黄止痛散)external application on acute gouty arthritis(AGA)of the shire Yunjie symptom(温热蕴结证)and its effect on serum IL-1βand TNF-αlevels.Methods:60 patients with AGA were randomly divided into the treatment group(30 cases)and control group(30 cases).The control group was treated with eloricoxib tablets.The treatment group was given the external application of Zhihuang Zhitong San.The pain score of VAS,UA,ESR,CRP,IL-1βand TNF-αbefore and after 7 days in both groups were observed and the efficacy was ovaluated.Results:After 7 days of treatment,the total efficiency in the treatment group was 96.67%,higher than 86.67%in the control group,with a statistically significant difference(Z>1.96,P<0.05).The pain and swelling scores in the treatment group were lower than those in the control group,with a statistically significant difference(P<0.05).The serum levels of UA,C-RP,IL-1βand TNF-αin the treatment group were lower than those in the control group(P<0.05).There was no statistical significant difference in ESR between the treatment group and control group(P>0.05).Conclusion:Zhihuang Zhitong San shows a significant effect on AGA,can significantly reduce the levels of IL-1β,TNF-αand other inflammatory factors.
