改良Blandy法行经尿道前列腺切除术对高危重度前列腺增生症患者手术效果、出血量及术后IPSS评分的影响

摘　　要：目的探讨高危重度前列腺增生症患者治疗中改良Blandy法行经尿道前列腺切除术的手术效果、出血量及术后IPSS评分的影响。方法选取本院2015年4月至2017年4月收治的300例高危重度前列腺增生症患者,依据经尿道前列腺切除术方法将这些患者分为Blandy法组(n=150)和改良Blandy法组(n=150)两组,对两组患者的术中术后指标、IPSS评分、术后并发症发生情况进行统计分析。结果改良Blandy法组患者的手术时间、术后住院时间均显著短于Blandy法组(P<0.05),术中出血量、输血量均显著少于Blandy法组(P<0.05),IPSS评分显著低于Blandy法组(P<0.05),术后并发症发生率6.0%(9/150)显著低于Blandy法组17.3%(26/150)(P<0.05)。结论高危重度前列腺增生症患者治疗中改良Blandy法较Blandy法行经尿道前列腺切除术的手术效果好,更能有效减少患者的术中出血量,降低患者的术后IPSS评分。Objective To investigate the effect of modified Blandy's transurethral prostatectomy in the treatment of high-risk patients with severe prostatic hyperplasia,the amount of bleeding and the influence of IPSS score after operation.Methods 300 patients with high-risk and severe prostatic hyperplasia admitted to our hospital from April 2015 to April 2017 were divided into Blandy method group(n=150)and improved Blandy method group(n=150)according to transurethral prostatectomy.The intraoperative and postoperative indexes,IPSS scores and complications of the two groups were compared.Statistical analysis was conducted.Results The operation time and hospitalization time of the improved Blandy method group were significantly shorter than those of the Blandy method group(P<0.05),the intraoperative blood loss and transfusion volume were significantly less than those of the Blandy method group(P<0.05),the IPSS score was significantly lower than that of the Blandy method group(P<0.05),and the incidence of postoperative complications was 6.0%(9/150)significantly lower than that of the Blandy method group(17.3%)(26/150)(P<0.05).Conclusion Modified Blandy's method is better than Blandy's method in the treatment of high-risk and severe prostatic hyperplasia.It can effectively reduce intraoperative bleeding and IPSS score of patients after operation.
