摘  要:【目的】探讨糖尿病视网膜病(diabetic retinopathy,DR)变患者血清、玻璃体中miRNA-200b、血管内皮生长因子(vascular endothelial growth factor,VEGF)的表达水平及临床意义。【方法】选取2016年2月至2017年2月我院收治的79例2型糖尿病(type 2 diabetes mellitus,T2DM)患者为研究对象,根据是否合并DR及病变程度将其分为增殖型DR组(PDR组)(27例)、单纯型DR组(BDR组)(30例)、单纯T2DM组(NDR组)(22例)3组,同期选取25例体检健康者为对照组(NC1组),选取7例因孔源性视网膜脱离行玻璃体切割术患者作为玻璃体研究对照组(NC2组),利用全自动生化分析仪检测各组血糖、血脂指标,采用qRT-PCR检测miRNA-200b表达水平,采用ELISA法检测VEGF水平。【结果】DR组空腹血糖、餐后2 h血糖、糖化血红蛋白、低密度脂蛋白胆固醇、总胆固醇、血清VEGF水平明显高于NC1组,差异有统计学意义(P<0.05),高密度脂蛋白胆固醇、血清miRNA-200b mRNA水平明显低于NC1组(P<0.05),且随着DR病情加重,空腹血糖、糖化血红蛋白、餐后2 h血糖、低密度脂蛋白胆固醇、VEGF呈明显递增趋势(P<0.05),高密度脂蛋白胆固醇、miRNA-200b m RNA呈明显递减趋势(P<0.05);PDR组玻璃体中VEGF水平明显高于NC2组(P<0.05),miRNA-200b mRNA水平明显低于NC2组(P<0.05);相关性分析结果显示,DR患者血清中miRNA-200b表达水平与糖化血红蛋白呈明显负相关(r=-0.426,P<0.05),VEGF水平与糖化血红蛋白呈明显正相关(r=0.579,P<0.05),miRNA-200b与VEGF表达呈明显负相关(r=-0.512,P<0.05)。【结论】DR血清、玻璃体中miRNA-200b呈明显低表达,VEGF明显高表达,二者血清中表达异常与DR患者血糖控制差、代谢紊乱密切呈正相关。【Objective】To explore the expression levels of miRNA-200b and vascular endothelial growth factor(VEGF) in serum and vitreous body of patients with diabetic retinopathy(DR) and its significance.【Methods】A total of 79 cases with type 2 diabetes mellitus(T2DM) in our hospital from February 2016 to February 2017 were selected as the study objects. According to whether DR was combined and the lesion extent, the patients were divided into proliferative diabetic retinopathy group(PDR group)(27 cases), simple diabetic retinopathy group(BDR group)(30 cases) and simple T2DM group(NDR)(22 cases). There were 25 cases of physical examination health selected as the control group(NC1 group), and 7 cases of undergoing vitrectomy for rhegmatogenous retinal detachment as the control group of vitreous study(NC2 group). Blood glucose and lipids were measured by automatic biochemical analyzer, and expression levels of miRNA-200b were detected by real-time fluorescent quantitative polymerase chain reaction(qRTPCR), and VEGF level was tested by enzyme linked immunosorbent assay(ELISA). 【Results】Fasting blood glucose(FPG), 2 h postprandial blood glucose(2 hPG), glycosylated hemoglobin(HbA1c), low density lipoprotein cholesterol(LDL-C), total cholesterol(TC), serum VEGF level in the DR group were significantly higher than those in the NC1 group, with statistically significant difference(P< 0.05). High density lipoprotein cholesterol(HDL-C), serum miRNA-200b and mRNA levels in the DR group were significantly lower than those in the NC1 group(P< 0.05). With the aggravation of DR, FPG, HbA1c, 2 hPG, LDL-C and VEGF showed an obvious increasing trend(P<0.05), and HDL-C, miRNA-200 b and mRNA showed a decreasing trend(P<0.05). VEGF level in vitreous tissue of PDR group was significantly higher than that in the NC2 group(P<0.05), and miRNA-200 b mRNA was lower than that in the NC2 group(P< 0.05). Correlation analysis showed that miRNA-200 b expression level in serum of DR patients was negatively correlated with HbA1c(r=-0.426,P<0.05)
