摘  要:目的探讨全球首例确诊的人感染H7N4禽流感病毒性肺炎的CT影像表现。方法对首例经临床及病毒学检查证实的人感染H7N4禽流感病毒性肺炎患者的临床资料及其CT影像表现进行观察分析。结果患者首次CT检查发现右肺下叶实变影,快速进展为两肺下叶片状实变影、磨玻璃影,两上中肺野斑片状磨玻璃影。治疗后,未见新发病灶,肺内斑片状磨玻璃影吸收,两肺下叶实变影范围缩小。结论人感染H7N4禽流感病毒肺炎主要CT表现为两下肺部实变影,两肺多叶段磨玻璃影。经奥司他韦抗病毒治疗后病灶吸收,CT检查对肺炎的动态变化、疗效观察有重要作用。Objective To investigate the CT scan demonstratons of the globally first case of human infected avian influenza virus(H7N4)pneumonia.Methods The clinical data and CT scan demonstrations of the globally first case of human infected avian influenza virus(H7N4)pneumonia were observed and analyzed.Results The first CT scan demonstrated consolidation opacity in the right lower lung lobe,which rapidly progressed into laminar consolidation opacity and ground glass opacity(GGO)in both lower lung lobes and patchy GGO in both upper and middle lung fields.After treatment,no new lesions were demonstrated.The intrapulmonary patchy GGO was absorbed and the consolidation opacity in both lower lung lobes shrank.Conclusion The main CT scan demonstrations of the globally first case of H7N4 virus pneumonia include consolidation opacity in both lower lungs,multi-lobes and multi-segment GGO in both lungs.After anti-viral treatment by Oseltamivir,the lesions are demonstrated to be absorbed.And CT scan plays important role in observing the dynamic changes of pneumonia lesions and assessing therapeutic efficacy.
