摘 要:目的分析甲状腺功能正常的冠心病患者ITM(颈动脉内膜中层厚度)与TSH(血清促甲状腺激素水平)的相关性。方法随机选取本院收治的冠心病(甲状腺功能正常)患者116例,2016年5月至2018年5月为研究时段,根据ITM分组,分为ITM增厚组与ITM正常组,对比两组的临床资料;其次根据TSH高组、TSH中组、TSH低组,对比3组的临床资料,分析TSH与IMT的相关性。结果 ITM增厚组的年龄、BMI、TG、TC、LDL-C、载脂蛋白B、载脂蛋白A1、TSH显著高于正常组(P<0.05),HDL-C水平显著低于正常组(P<0.05);ITM增厚组与ITM正常组性别相比差异无统计学意义。TSH高组IMT、TC、TG、BMI水平显著高于TSH中组、TSH低组(P<0.05);TSH高组、TSH中组、TSH低组性别、年龄、HDL-C、LDL-C、载脂蛋白B、载脂蛋白AI两组相比差异无统计学意义。血清TSH与IMT仍旧存在正相关性(r=0.186,P<0.05)。TG、TSH、年龄是影响IMT的独立危险因素(P<0.05)。结论冠心病(甲状腺功能正常)患者机体TSH水平与ITM呈正相关性,TSH水平是引发ITM的独立危险因素之一,在临床中具有较高的参考价值。Objective To analyze the correlation between ITM(carotid intima-media thickness)and TSH(serum thyroid stimulating hormone)in patients with coronary heart disease with normal thyroid function.Methods A total of 116 patients with coronary heart disease(normal thyroid function)admitted to our hospital were randomly selected.From May 2016 to May 2018,the study period was divided into ITM thickening group and ITM normal group according to ITM group.Clinical data;Secondly,according to the TSH high group,TSH middle group,TSH low group,and the clinical data of the three groups,the correlation between TSH and IMT was analyzed.Results The age,BMI,TG,TC,LDL-C,apolipoprotein B,apolipoprotein A1,and TSH in the ITM thickening group were significantly higher than those in the normal group(P<0.05),and the HDLC level was significantly lower than that in the normal group(P<0.05).There was no significant difference in the ITM thickening group compared with the normal ITM group.The levels of IMT,TC,TG and BMI in TSH group were significantly higher than those in TSH middle group and TSH low group(P<0.05).TSH high group,TSH middle group,TSH low group sex,age,HDL-C,LDL-C,There was no significant difference in apolipoprotein B and apolipoprotein AI between the two groups.There was still a positive correlation between serum TSH and IMT(r=0.186,P<0.05).TG,TSH,and age were independent risk factors for IMT(P<0.05).Conclusion The TSH level of patients with coronary heart disease(normal thyroid function)is positively correlated with ITM.TSH level is one of the independent risk factors for ITM,and it has high reference value in clinical.
