晚期癌症患者优逝期望现状调查及影响因素分析

摘　　要：目的对晚期癌症患者优逝期望现状进行调查,并分析影响其优逝期望的主要因素,为临床开展优逝护理提供循证依据。方法选择2016年1月至2018年1月本市某三级甲等医院肿瘤科收治的晚期癌症患者56例,运用一般资料调查表、中文版优逝期望量表进行现状调查。结果晚期癌症患者优逝期望值均分为77. 14分;调查对象对身心舒适、在喜欢地方逝去、不成为负担、良好的家庭关系、独立自理、自然死亡、临终前的准备、对将来的把控,共8项优逝期望值较高;对维持希望和快乐、良好的医患关系、环境舒适、获得个人尊重、自尊和美丽,共5项优势期望值中等,对个人使命的完成、接受足够的治疗、宗教信仰和精神慰藉、自觉意识、有人生价值,共5项期望值较低。不同年龄、文化程度、自知患病时长、职业、是否有危重患者接触史对晚期癌症患者优逝期望值影响差异具有统计学意义(P <0. 05);其中年龄、文化程度、自知患病时长、是否有危重患者接触史为主要影响因素。结论晚期癌症患者并未避忌谈论优逝,在喜欢的地方、没有疼痛、在亲人的陪伴下、安详而有尊严地离世是其共同的优逝期望;临床开展优逝护理应重点对高龄、文化程度偏低、患病时长短、没有接触危重患者史的患者进行潜移默化地优逝影响。Objective To investigate the current status of expectation for hospice care among terminal cancer patients,and to analyze the main factors affecting their expectation,thus providing evidence for clinical development of nursing for hospice care.Methods A total of 56 patients with advanced cancer admitted to the oncology department of a grade III class A hospital in the city from January 2016 to January 2018 were selected.The general information questionnaire and the Chinese version of the expectation for hospice care scale were used for the current situation investigation.Results The average score of expectation for hospice care of terminal cancer patients was 77.14 points.These study subjects had high expectation for the following 8 aspects of hospice care,including the comfortable body and mind,died in the place they liked,did not become a burden,good family relationship,independent self-care,natural death,preparation before death,and control over the future.These study subjects had mediate expectation for the following 5 aspects of hospice care,including maintaining hope and happiness,good doctor-patient relationship,comfortable environment,personal respect,as well as self-esteem and beauty.These study subjects had low expectation for the following 5 aspects of hospice care,including the completion of personal mission,receiving adequate treatment,religious beliefs and spiritual comfort,conscious awareness,and life value.Different ages,education level,self-awareness of illness,occupation,and whether there was a history of exposure to critically ill patients had statistically significant differences in the expected outcomes of hospice care among patients with advanced cancer(P<0.05).The age,education level,awareness time of cancer and exposure history of critically ill patients were the main influencing factor.Conclusion Patients with advanced cancer have not avoided talking about the hospice care.Dying in the place they like without pain,and with dignity and peace,as well as accompanied by their loved ones,are t
