喉癌患者术后下呼吸道感染危险因素分析及护理策略

摘　　要：目的探讨喉癌患者术后下呼吸道感染的危险因素及护理策略。方法选取我院2016年2月至2018年2月92例接受喉癌手术治疗的患者作为本次调查对象,统计患者性别、年龄、吸烟史、饮酒史、手术时间、手术方式、是否使用呼吸机、是否留置胃管、是否输血、应用抗菌药物时间、有无肺部疾病等资料,并进行单因素分析和多因素logistic回归分析。结果本次接受调查的患者中下呼吸道感染11例(11. 96%),经多因素logistic回归分析发现,手术时间、手术方式、留置胃管、应用抗菌药物时间、肺部疾病为影响喉癌患者术后下呼吸道感染的独立危险因素。结论手术时间、手术方式、留置胃管、应用抗菌药物时间、合并肺部疾病为喉癌患者术后发生下呼吸道感染的独立危险因素,择期喉癌手术患者应选择合适的手术方式,术中护理人员与术者紧密配合,缩短手术整体时间,且术后患者应合理使用抗菌药物,尽早拔出胃管、撤离呼吸机,恢复肺部自主呼吸功能。Objective To explore the risk factors for lower respiratory tract infection in patients with laryngeal cancer and nursing strategies.Methods To collect data of gender,age,smoking history,drinking history,operation time,operation method,whether use ventilator,whether keep gastric tube,whether have blood transfusion,time of using antibacterial drugs,and whether have lung disease,and performed univariate analysis and multivariate logistic regression analysis,92 cases of patients with laryngeal cancer who accepted surgery in our hospital from February 2016 to February 2018 were observed.Results Among the 11 patients (11.96%)with lower respiratory tract infections,the multi-factor logistic regression analysis showed that operation time,operation method,indwelling gastric tube,time of using antibacterial drugs and lung disease were independent risk factors for postoperative lower respiratory tract infection in patients with laryngeal cancer.Conclusion Operation time,operation method,indwelling gastric tube,time of using antibacterial drugs and pulmonary disease are independent risk factors for postoperative lower respiratory tract infection in patients with laryngeal cancer.Patients of appointed laryngeal cancer surgery should choose the appropriate surgical approach.Intraoperative nursing staff should work efficiently with the surgeon to shorten the overall time of surgery.Postoperative patients should use antibiotics reasonably,pull out the stomach tube as soon as possible and evacuate the ventilator to restore the lung's spontaneous breathing function.
