摘　　要：目的探讨血管内皮生长因子(VEGF)联合妊娠相关血浆蛋白A(PAPP-A)诊断异位妊娠的临床价值。方法选取209例异位妊娠患者(EP组)、183例先兆流产患者(TA组)、200例正常妊娠孕妇(对照组),所有研究对象均采用酶联免疫吸附试验(ELISA)检测其血浆VEGF、PAPP-A。结果⑴3组研究对象VEGF、PAPP-A相比差异有统计学意义(P<0.05);EP组VEGF显著高于TA组、对照组(P<0.05),PAPP-A显著低于TA组、对照组(P<0.05);TA组VEGF显著高于对照组(P<0.05),PAPP-A显著低于对照组(P<0.05)。⑵受试者工作特征曲线(ROC)分析显示,VEGF、PAPP-A诊断异位妊娠的最佳阈值分别为290.8 pg/ml、6.91 ng/ml,ROC曲线下面积分别为0.863、0.807。VEGF联合PAPP-A诊断异位妊娠的灵敏度及准确度显著高于VEGF、PAPP-A(P<0.05),VEGF诊断异位妊娠的灵敏度及准确度显著高于PAPP-A(P<0.05)。结论 VEGF联合PAPPA在鉴别诊断异位妊娠与先兆流产方面具有良好的临床实用价值。Objective To investigate the clinical value of vascular endothelial growth factor(VEGF)combined with pregnancy related plasma protein A(PAPP-A)in the diagnosis of ectopic pregnancy.Methods A total of 209 patients with ectopic pregnancy(EP group),183 patients with threatened abortion(TA group)and 200 normal pregnant women(control group)were selected,and the plasma VEGF and PAPP-A were detected by enzyme linked immunosorbent assay(ELISA)in all subjects.Results⑴There were statistical differences in the VEGF and PAPP-A among the 3 groups(P<0.05).The VEGF in EP group was statistically higher than that in TA group and control group(P<0.05),while the PAPP-A was statistically lower than that in TA group and control group(P<0.05).The VEGF in TA group was statistically higher than that in control group(P<0.05),while the PAPP-A was statistically lower than that in control group(P<0.05).⑵The receiver operating characteristic curve(ROC)analysis showed that the best thresholds of VEGF and PAPP-A in the diagnosis of ectopic pregnancy were 290.8pg/ml,6.91ng/ml,and the area under ROC curve were 0.863 and 0.807,respectively.In the diagnosis of ectopic pregnancy,the sensitivity and accuracy of VEGF combined with PAPP-A were statistically higher than those of VEGF and PAPP-A(P<0.05),and the sensitivity and accuracy of VEGF were statistically higher than those of PAPP-A(P<0.05).Conclusions The VEGF combined with PAPP-A has good clinical value in differential diagnosis of ectopic pregnancy and threatened abortion.
