曼月乐保守治疗子宫内膜异位症的疗效及对子宫动脉血流动力学、血清EMAb及激素水平的影响

摘　　要：目的探讨曼月乐保守治疗子宫内膜异位症的临床疗效及对子宫动脉血流动力学、血清抗子宫内膜抗体(EMAb)及激素水平的影响,为不愿手术的子宫内膜异位症患者提供参考。方法选择子宫内膜异位症患者540例,随机分为观察组和对照组,各270例,其中对照组患者给予达菲林治疗,观察组患者给予曼月乐治疗,观察两组患者治疗前后的月经量、痛经情况差异,比较两组患者治疗前后血清雌二醇(E2)、孕酮(P)、卵泡刺激素(LSH)、黄体生成素(LH)、促肾上腺皮质激素(ACTH)等激素水平及血清EMAb水平的差异,分析两组患者治疗前后左右子宫动脉的最大血流速度、搏动指数、阻力指数差异,考察两组患者治疗后的临床疗效差异。结果与治疗前相比,两组患者治疗后月经量、VAS评分以及血清EMAb水平明显下降;血清ACTH、LH、FSH水平升高,血清P和E2水平降低;左右子宫动脉的阻力指数、搏动指数显著下降,最大血流速度显著升高;且观察组患者的相关指标改善更为显著(P<0.05)。观察组患者的治疗有效率明显高于对照组(P<0.05)。结论曼月乐保守治疗子宫内膜异位症可以明显改善患者的临床症状、子宫动脉血流动力学指标,增强血清FSH和LH激素的分泌,降低血清EMAb、P、E2水平,保护卵巢功能,增强临床疗效。Objective To investigate the clinical efficacy of conservative treatment of endometriosis with Mirena and its effects on uterine artery hemodynamics,serum EMAb and hormone levels,and to provide reference for patients with endometriosis who do not want surgery.Methods A total of 540 patients with endometriosis who were admitted were randomly divided into two groups which were observation group(270 cases)and control group(270 cases).In the control group,patients were given Tamiflu,patients in the observation group were treated with Mirena,and menstrual flow and dysmenorrhea were observed before and after treatment.The levels of serum estradiol(E2),progesterone(P),follicle stimulating hormone(LSH),luteinizing hormone(LH)and adrenocortico tropic hormone(ACTH)were compared between the two groups before and after treatment.The levels of serum estradiol(E2),progesterone(P),follicle stimulating hormone(LSH),luteinizing hormone(LH),adrenocorticotropic hormone(ACTH)and anti endometrial antibody(EMAb)were compared between the two groups before and after treatment.Analysis of the two groups before and after treatment on both sides of the uterine artery maximum blood flow velocity,pulsatility index,resistance index differences,two groups of patients treated after treatment differences in clinical efficacy.Results Compared with before treatment,menstruation volume,VAS score and serum EMAb level decreased significantly in both groups after treatment.Serum levels of ACTH,LH and FSH increased and serum P and E2 levels decreased.The resistance index and pulsatility index of left and right uterine arteries decreased significantly,and the maximum blood flow velocity increased significantly.However,the relative indexes of observation group improved and there were statistical significantly differences(P<0.05).In addition,the observation group of patients with treatment efficiency was significantly higher than the control group and there were statistical significantly differences(P<0.05).Conclusion Conservative treatment of endometrios
