摘 要:目的评价右美托咪定预处理对大鼠肠缺血再灌注损伤(IRI)的影响。方法健康成年雄性SD大鼠54只,体重270-310g,随机分成3组:假手术组(Sham)、肠缺血再灌注损伤组(I/R组)、右美托咪定+肠缺血再灌注损伤组(DL组)。采用夹闭肠系膜上动脉根部45min,再灌注2h制备大鼠肠缺血再灌注损伤模型。DL组在缺血前30min经大鼠尾静脉泵注右美托咪定2.5μg·kg^(-1)·h^(-1),Sham组和I/R组给予等容量生理盐水。于再灌注2h采用免疫组织化学的方法检测大鼠血管内皮细胞NF-κB及ICAM-1表达,同时检测血浆超氧化物歧化酶(SOD)活力及丙二醛(MDA)含量。结果与Sham组比较,I/R组和DL组血管内皮细胞NF-κB及ICAM-1表达升高,血浆MDA水平升高,SOD活力降低(P<0.05);与I/R组比较,DL组血管内皮细胞NF-κB及ICAM-1表达下降,血浆SOD活力升高,MDA水平降低(P<0.05)。结论右美托咪定预处理可减轻肠缺血-再灌注损伤,其机制可能与其抑制NF-κB活性,降低ICAM-1表达有关。Objective The effect and mechanisms of dexmedetomidine pretreatment on intestinal I/R injury in mice.Methods Intestinal model of ischemia/reperfusion injury was established,54 male SD rats,weighing 270 to 310g,and the animals were divided into three groups:sham group,ischemia/reperfusion group and dexmedetomidine pretreatment group.Using clipping root 45min,the superior mesenteric artery 2h reperfusion intestinal ischemia-reperfusion injury model.The DL group was treated with 2.5ug·kg^-1·h^-1 dose of dexmedetomidine hydrochloride in the 30min caudal vein before ischemia,and the sham operation group,the model group was given normal saline of equal volume.Effects of dexmedetomidine on intestinal I/R injury was evaluated by superoxide dismutase(SOD)and malondialdehyde(MDA),and then detected the levels of NF-κB,ICAM-1 by immunohistochemistryat Reperfusion for 2 hours.Results Compared with the sham operation group,I/R group and DL group increased the level of MDA,Decreased SOD vitality(P<0.05);compared with I/R group,DL group,The activity of SOD in group DL was elevated,and the level of MDA was decreased(P<0.05);compared with the sham operation group,I/R group,NF-kappa B,ICAM-1 content increased significantly(P<0.05);compared with I/R group,DL groupNF-κB,ICAM-1 concentration level decreased(P<0.05).Conclusion Dexmedetomidine pretreatment can inhibition of oxidative stress and release of inflammatory factors induced by intestinal ischemia reperfusion,and then inhibit the expression of NF-κBand ICAM-1,thereby reducing the extent of intestinal ischemia-reperfusion injury.
