右美托咪定对老年患者关节置换术后早期谵妄的预防效果

摘　　要：目的分析采用右美托咪定对老年患者关节置换术后早期谵妄的预防效果。方法 2016年2月—2017年9月,于该院行髋关节、膝关节置换术的老年患者中选取98例,随机分为两组,于手术开始前30 min内给予观察组患者右美托咪定静脉泵注,给予对照组患者等量生理盐水静脉泵注,结束手术后,两组患者均行PCIA(自控静脉镇痛),对比两组不同时间点生命体征变化情况、VAS(视觉模拟疼痛)评分、术后谵妄发生情况。结果两组患者术后第1、2、3天时的VAS评分对比,组间差异无统计学意义(t=0.758 5、0.168 7、0.096 2,P>0.05);观察组术后谵妄发生率为4.08%(2/49),低于对照组的28.57%(14/49)(χ~2=8.632 5,P=0.005 8);观察组谵妄持续时间为(1.32±0.06)d,短于对照组的(3.68±0.95)d(t=12.562 8,P=0.000 0)。结论采用右美托咪定能有效预防老年患者关节置换术后早期谵妄,可推广。Objective To analyze the preventive effect of dexmedetomidine on early sputum after joint replacement in elderly patients.Methods From February 2016 to September 2017,98 patients were selected from the elderly patients who underwent hip and knee arthroplasty in the hospital.They were randomly divided into two groups.The patients in the observation group were given dexmedetom within 30 minutes before the start of surgery of intravenous pumping,the control group was given the same amount of saline intravenous pumping,after the end of the operation,both groups of patients underwent PCIA(self-controlled intravenous analgesia),comparing the changes in vital signs at different time points,VAS(visual simulated pain)score,postoperative delirium.Results There were no significant differences in VAS scores between the two groups on the 1st,2nd,and 3rd postoperative days(t=0.758 5,0.168 7,0.096 2,P>0.05).The incidence of postoperative delirium in the observation group was 4.08%(2/49),lower than the control group 28.57%(14/49)(χ^2=8.632 5,P=0.005 8);the duration of the observation group was(1.32±0.06)d,which was shorter than that of the control group(3.68±0.95)d(t=12.562 8,P=0.000 0).Conclusion The use of dexmedetomidine can effectively prevent early sputum after joint replacement in elderly patients and can be promoted.
